00:00Juan Adams is a 47-year-old from LA whose weight has become a problem that he can't ignore anymore.
00:06Currently, I live with my sister and my nephew and my niece.
00:12I've been living with my sister off and on since losing mobility.
00:17My mother and my brother come down to Metairie. We do a lot together.
00:22Juan hates not being able to do anything on his own,
00:25and he has to depend on his sister and her children to get him through the day,

00:30Of course, I have concerns about his weight gain because he's my favorite person.
00:36I want him to be here with me as long as possible.
00:39But I've always been protective of him ever since we were kids.
00:43So I try to check on him every single day on the phone,
00:46or I come physically to check on him every single day.
00:50I'll tell you what I have and you tell me what you are like.
00:52We have eggs, grits, bacon, and sauces.
01:01But it's not hard to see that even his family is growing concerned about the pounds Juan has put on.
01:06The type of person he is, you know, he wants everything.
01:10And if I tried to cut back, he would say, no, Mom.
01:13How many eggs, Juan?
01:15Just cook it all.
01:17When he wants something, he's real persistent.
01:21So, you know, I don't like it, but to come to the grip that this is what's going to happen.
01:29My mom, she's a down-home Southern Cajun Creole cook.
01:34And I love being around my mom's cooking.
01:37That's the food I'm addicted to.
01:38My brain just gets fixated on all the smells.
01:42But then again, his family members are also his biggest enablers,
01:45and they give Juan all the junk food that he wants to eat.
01:48So because we were so young, you know, we couldn't do anything about it.
01:52I felt angry a lot. I felt sad.
01:56I don't know why, but, you know, eating a candy bar just made me feel better.
02:01It just always brought me comfort, you know.
02:04And from that point on, it became my habit.
02:07Before I talked to anybody else about it, food was the first thing that I would do.
02:12I think I really started to notice.
02:14I was overweight when all of the little kids started, you know, calling me fat.
02:19You can tell that Juan holds himself responsible for not being able to take care of his mom,
02:24but what could he have even done as a child?
02:26You want some more juice?
02:28Yeah. Then when I was around 11, my mom got to a point with my dad
02:32when she finally made the decision to leave.
02:34Once they got divorced, I spent the majority of my time with my mom.
02:38I couldn't really ask for a better mom, so…
02:41But to fight all this helplessness,
02:43he turned to food because that's the only thing that made him feel okay.
02:46I would cope by eating more.
02:50After the divorce, our lifestyle changed drastically.
02:56We didn't have as much means as we had before.
02:59When I ate, I would use the excuse that I wouldn't know when I would eat again,
03:03so I would overeat. I guess I was maybe around 230 pounds.
03:09By the time I was in middle school, carrying that much weight became impactful
03:13because my whole family was into sports. And up until that point, I had been really active.
03:19I had grown up playing little leads.
03:21The worst part is that Juan recognizes how destructive his emotional dependence on food is,
03:26but he has this insatiable hunger that he just can't satisfy, no matter how much he eats.
03:30He always loved to eat, but more than the portion sizes,
03:36I think it's the amount of times that he's up snacking and stuff like that.
03:42I started out pursuing the music career,
03:45but not many people who look like me are very successful in that line of work.
03:51So I got licensed as a minister, I got ordained, but then somewhere along the way,
03:57my increasing weight began to affect my mobility.
04:01Until one day I fell, and it was hell getting me up.
04:07He's at a point where he only looks forward to eating as soon as he wakes up,
04:10and as soon as he finishes one meal, he starts thinking about the other.
04:14He was like weak, and he said he was overheating and stuff. His body was getting hot.
04:19I didn't know what to think at that moment.
04:23At that point, I figured it wasn't safe to live on my own.
04:27Okay, Juan. Are you ready?
04:32I'm going to go to the grocery store.
04:34So what are we going to buy?
04:36I mean, crawfish rice, shrimp rice, mac and cheese, something.
04:42Okay.
04:43And Juan likes to laugh it all off, but his family is completely heartbroken at how
04:48difficult his life has become just because of his love for food.
04:51My brother's the only one who have a car that's big enough for me to fit in.
04:55There's a lot of things that if I would have known, I would have done better.
04:59If I would always tell him, like, slow down a little bit, but I did it in a loving way.
05:05It's hard for me to tell him no, because I like to see him happy.
05:10You can look at it as me being an enabler, but I look at it as just helping out my brother.
05:18And no matter how hard it is for him to get up and walk, he never misses a chance to go to the
05:23grocery store and grab his favorite snacks.
05:25It's getting harder for him to move around and to do the things that he would like to do.
05:32And now he's gotten to the part where he's disgusted. And he's telling himself that,
05:40hey, it's either this or my life. But I always have hope that he's coming out of this.
05:49Despite his family's concern, though, none of them can say no to Juan as he exclusively eats the
05:53unhealthiest things one can ever imagine.

05:57I'm willing to push him in the right direction and assist him instead of assisting him
06:03in doing all the wrong things, assisting him in doing the right things.
06:08I know I can't live forever at this size. My physical body is breaking down on me.
06:14I just know that I have to make some changes. If I don't get help, I think I'm very close
06:21to becoming the total shelter.
06:24Juan's day ends with a huge fast food meal, an entire pizza all for himself,
06:28and that is just not the way anyone is supposed to be living.
06:31All right. Ready for your little journey, huh? Ready to see Dr. Niles?

06:38This is the beginning of a new beginning, huh? That's what we gonna call it?
06:44Yep, definitely a new beginning.
06:47Today I'm taking a leap of faith and I'm driving to Houston, Texas to meet a man named Dr. Niles Zorden.
06:54Who works with obese individuals whose lives are out of control from their addiction to food.
07:00So, he thankfully makes the decision to see Dr. Niles and try to get his weight under control before
07:05it's too late.
07:06It's gonna be a long drive that it will be unpleasant. This is a route that I know pretty
07:11well because we lived in Houston after Hurricane Katrina. And I've been back and forth a lot,
07:15but my health is worse than it's ever been. And it means that this ride will also be worse.
07:21There's only one good thing that comes to mind, which is one of my favorite restaurants that will
07:25pass along the way.
07:27But Juan is nervous about the long journey ahead, especially because he hasn't traveled so far in a
07:31very long time.
07:55At the same time, Juan knows that he needs to do this if he ever wants to get better.
07:59But a few hours into the road trip, the only thing he is craving is a greasy,
08:03cheesy fast food meal.
08:05Oh, man.
08:07We've been on the road for a couple of hours and my knee pains is just miserable.
08:11So we're gonna have to stop.
08:12But the good news is we've made it to my favorite restaurant.
08:16Let me get one pork boudin, three seafood, two crawfish balls, two pepper jack boudin balls.
08:30What's the roll-up things?
08:31The eight roll-up?
08:33Crawfish roll-up.

08:35Like four.
08:36Soon enough, his body starts hurting and they have no choice but to stop at a fast food restaurant where
08:41Juan can indulge in his addiction once again, which is a horrible way to kick off his
08:45journey.
08:46These things good, man.
08:48Oh, yeah.
08:50But even if I wasn't going to weigh in tomorrow, I know better than to do that.
08:54So it's probably another two or three hours on the road.
08:58Man, you ready to get back on the road now?
09:00Yeah, yeah.
09:02I'm ready.
09:02Let's do it.
09:03After his meal has finished, Juan feels a pang of guilt.
09:06But that doesn't last very long because that's just the way his brain works.
09:10Thank God we made it to Houston, Texas.
09:12That was even worse than I thought.
09:14And then my pain is maxed out.
09:15Tomorrow, I'm going to meet Dr. Niles Zarden.
09:18And I'm nervous and scared to see how much I weigh.
09:21Only thing is, now I have to make it to our room.
09:24My legs are swollen.
09:25My knees are swollen.
09:26So because of that, it caused, you know, a lot of pain during the ride.
09:31After a few more hours of driving, Juan and his family are in Texas.
09:34But the whole journey has been extremely hard on his body.
09:37And Juan has no idea how to survive this pain.
09:40I'm at Dr. Niles Clinic in Houston to see how much I weigh and to get his help losing weight.
09:45It's been a few years since I got a weigh-in, so I'm nervous about that number.
09:53This is the day I've been waiting for.
09:56And I think I'm also, you know, a little nervous about just the whole experience,
10:01but excited at the same time.
10:05This is it.
10:06The next morning, it's finally time for Juan to see how much he weighs.
10:10And it's been a while since he has stepped on the scales, so he has no idea how bad things are.
10:16Yeah, it's been that way for a while now.
10:20That's why I really need Dr. Niles' help.
10:22My only concern is that I waited too long to make the call.
10:26And that he's going to tell me it's already too late for me.
10:30I'm just praying he knows how to help.
10:32He holds his breath as the scales reveal that he weighs over 627 pounds,
10:36and Juan is extremely disappointed to learn how much he weighs.
10:39He just hopes Dr. Now agrees to help him.
10:42So, Juan, what can I do for you today?
10:44I came to try to make some life-altering changes.
10:51To lose a few hundreds, gain my mobility back.
10:56So, what kind of issue you got with your mobility?

11:02600 pounds of weight definitely is going to make that much worse.
11:06Do you do any activity that keeps you mobile?
11:10I'm a pastor, and I do music and, you know.
11:14Pastor?
11:15Yes, sir.
11:16Did the Lord tell you that your body is your temple?
11:20Yeah, he did, and it's my issue.
11:22It's my struggle.
11:23Of course, Dr. Now is a little shocked to learn how much Juan weighs,
11:26and he wants him to realize that he has a long road ahead.
11:29Just say, walk me through a typical day.
11:32What do you eat from the time you wake up till you go to bed?
11:35I would start eating maybe around 2.
11:37What do you eat at 2?
11:39Baked chicken, green beans.
11:41Uh, so you eat that at home?
11:44Yes.
11:44Who make that?
11:46Uh, well, my mom makes a lot of my meals and package it and bring it to me.
11:52All right, so your mom is your enabler.
11:55Is he blaming you for anything?
11:57No, I blame me.
12:00I am not.
12:00Hmm?
12:01I am not the enabler.
12:05But you bring in all the food.
12:09With abrigo, he's gonna eat it.
12:11By talking to Juan, Dr. Now realizes that his eating habits are not going to be easy to get rid of.
12:16So she brings you a week worth of food and you eat it all at once?
12:21Yeah.
12:21Then after that, do you snack on what?
12:24Uh, pickles, nuts.
12:27Sometimes I do have like sweet vengeance.
12:30I try to stay away from them, but sometimes I crave them.
12:33Pies, cakes.
12:34Pies and cake.
12:36Yeah.
12:37You don't lose weight and you go and buy a pie and a cake and then you have difficulty walking.
12:45Your trying is not very good.
12:48All right.
12:49So, you know, ultimately weight loss surgery is going to be a powerful tool for you to lose weight.
12:56But in order to get to that point, you need to lose some weight and you need to show me that you're
13:02willing to start to make the changes you need for the surgery to be successful, okay?
13:08And while Juan and his family are joking around about his weight,
13:11doctor now needs them to realize the urgency of his situation.
13:14Juan seems to spend a lot of time convincing himself.
13:17He is trying to be healthy and trying gimmicks to lose weight while continuing to eat whatever he wants.
13:27And it is clear that there is also a family dynamic that revolves around food.
13:34So there are a lot of changes that Juan and his family have to make if it's going to be possible
13:41to turn his situation around.
13:44Hopefully, this will change everybody's life.
13:46The doctor gives him two months to hit his weight goal,
13:49but he's not too sure if Juan will be able to do it,
13:51especially because of how unhealthy his family dynamic is.
13:54But for now, it looks like Juan is ready to turn his life around
13:57and start becoming a healthier version of himself.

14:04What you making?
14:05I'm gonna cook some egg omelet.
14:07Would you like one?
14:09Yeah.
14:10Would you like some cheese?
14:12Yeah, just, you know, the right amount, fat-free.
14:15Yes.
14:16It's been a couple weeks since I saw Dr. Now.
14:20And I've already made a lot of changes to my life and my diet.
14:24So I can try to reach his goals that he set for me to lose weight.
14:27I went over everything Dr. Now gave me with the diet and exercises.
14:32And I've been trying to follow them and adapt everything to how I'm supposed to be now.
14:37It's better than my omelet.
14:38I wonder why.
14:41In the second month of his weight loss journey, he has been making changes to his diet.
14:45But at this point, you can't tell if it's been enough.
14:48My family's been supportive too.
14:50And they're trying to help me eat better and encourage me to work out.
14:54And that's helping me motivate myself and keep going forward.
14:58Even when it feels like I'm too tired and too much pain to do exercises.
15:04Or when the cravings hit me really hard and all I want is something sweet.
15:09So I'm doing my best and giving it my all.
15:11And I gotta believe in myself and believe that my best is good enough.
15:16It hasn't been easy.
15:17But every time I make a sacrifice or do a workout that may be painful,
15:21I tell myself I'm one step closer to the goal of reaching a better life.
15:25But one thing is obvious.
15:26He has been eating way less than what he has become used to.
15:29At the same time, he has also been taking time out to stretch his muscles
15:33and work out to regain his mobility.
15:35And Juan has really been enjoying that.
15:37So how does your body feel?
15:41I feel good. I'm getting better.
15:44Getting better and better.
15:47You feel like you've lost the amount that you should have lost?
15:50I think so.
15:52I'm back to Dr. Nile to get my checkup and see how much weight I lost.
15:56It's been tough lately cutting back like I need.
15:59But I've been doing everything like I should.
16:02By the next month though, it's time to find out if all his hard work has paid off
16:06and Juan is extremely nervous as he heads to Houston.
16:08This whole time, I've been feeling like I've been doing pretty good.
16:12And now that we're here, all of a sudden I'm scared that I didn't do enough.
16:17If I fail, there's no one to blame but me.
16:19But I guess I won't have to if it's already too late.
16:22And my food addiction kills me.
16:24He believes that he has hit the goal he was supposed to,
16:27but the moment of truth comes as he steps onto the scales.
16:29That's short. I'm not feeling too good about that. I'm disappointed.
16:36And I bet the doctor will be too. I just hope he doesn't lose faith in me just yet,
16:41because I know I can do better.
16:42So what's going on with you? You lost nine pounds?
16:47Unfortunately.
16:48So what happened?
16:49I don't know. I thought I was following the diet and doing everything I was supposed to be doing,
16:55but I guess I didn't. I made changes, eliminating the carbs and the sugar,
17:01but I guess I didn't do enough.
17:03And while Juan's goal was to lose 70 pounds in two months,
17:05he has only managed to lose nine, which is obviously disappointing for him,
17:09his family, and Dr. Now, who cannot believe Juan's lack of progress.
17:12So you can taste the food, and that's about it, but not to use them as a dessert or part of your meal.
17:21And well, did you understand all the instructions we gave you in the paper?
17:26Well, I thought I did, but...
17:29I think the basic health eating habit is what the whole family needs to do.
17:36So we get to talk to nutritionists to understand what's good and what to do with your eating habit.
17:42And they go over all the stuff that you used to eat and help you with that.
17:48That's great.
17:48Okay? And you too?
17:50Yeah.
17:53Dr. Now immediately calls Juan out for not sticking to his diet,
17:56because if he had been eating healthy, there's no way he wouldn't have lost weight.
18:00Losing something is better than nothing, but losing nine pounds in two months is nothing.
18:06So Juan may have cut back on some of his unhealthy eating habits, but he's still either eating things
18:15that are not on the diet or not controlling his portion sizes.
18:20He does seem authentic to want to do better.
18:23So the issue may potentially be that he and his family doesn't fully understand all the changes they
18:30need to make with his diet. But no matter the reasons behind his own healthy habits,
18:37ultimately it's up to him to figure out what he needs to do.
18:41Dr. Now is willing to give Juan another chance, but he needs his family to stop enabling his addiction
18:46and help him develop a healthier lifestyle this time.
18:49I'm not feeling too good about today. I've been trying hard and my family's made a lot of sacrifices to
18:55help me. And I know this result is a letdown for all of them. But Dr. Now is giving me another
19:01chance to make it. So that's exactly what I'm going to do.
19:05This is Juan's last time to show the doctor that he is committed to this,
19:08and he is hoping that he's able to hit all the goals he needs to.
19:11Juan leaves with all the motivation he needs to push himself harder than before.
19:16I'm feeling pretty excited about seeing a nutritionist. I'm hoping to
19:19just get some questions answered and to find out if I've been on the right track, you know, pretty much.
19:28All right, Juan. So tell me a little bit about your goals. What is it exactly that you're wanting to achieve?
19:34I think I've been doing okay since I met Dr. Now, but you know,
19:39I'm just trying to make sure that I'm on the right track, doing the right things,
19:43uh, implement, you know, whatever I need to implement to keep progressing and, you know,
19:48even take it to another level. Okay. To kickstart the process, he is meeting with a nutritionist to
19:53see exactly what changes he needs to make to his diet. Okay. So I think that we need to have places for
19:59Juan's food and then places for the other's food. So some things, I mean, things like cereal and cookies,
20:08uh, should probably be, I'm assuming that's not for Juan. So probably should be in a place,
20:13in a separate place so that we can separate things. So when he comes in, what he's looking at
20:19are the things that are all ready for him to eat. So essentially you'd want to focus on having mostly
20:24vegetables, um, nuts and seeds. So pumpkin seeds, um, sunflower seeds, those kind of things would be okay.
20:31I'm going to take the corn out. Pasta sauce. Okay. The gumbo I'll take out.
20:39As hard as it is for Juan to give up food, he is serious this time around and he is willing to listen
20:44to the nutritionist's advice about how to power through his cravings instead of giving into them.
20:48Having family and friends support, you know, is always beneficial. So I'm really glad that
20:55they're even trying to come on this journey with me. Um, we have these yogurts that have the little
20:59candies on top. Probably not the greatest idea. Definitely our fruits we can put up here. Those
21:04are some good ones. Okay. So as far as the way that both the pantry and the refrigerator are set up,
21:11we want to make sure that what you see are only the things that you are allowed to have. So we want
21:18whenever you open up either the pantry or the refrigerator or the freezer to be only things that
21:23are checkmark okay for you to have. Okay. It's honestly shocking for Juan to see how much junk food is
21:29available to him from the comfort of his home and he is ready to cleanse himself of everything that
21:33has been holding him back. I'm a little nervous, but I'm excited to see the progress of what I've
21:39been doing. I've worked harder. My only problem is I got to get into a better routine. You know,
21:46naturally I've always been a night owl. I'm back at Dr. Now's clinic with my family for my checkup.
21:54And I'm feeling pretty good about how this one is going to go.
21:57In the fifth month, Juan is heading over to Houston to see if all his hard work has paid off and some
22:01part of him knows that he hasn't hit his goal. So if I did that, I'll be down to 548 today.
22:08And I really hope I did that. Your weight today is 601. But I still thought it would be more
22:15because I'm doing everything I can and working my hardest. So I'm not sure why the changes aren't more
22:21dramatic. So that leaves me disappointed because even though it feels better, it's still not a
22:27result I can bring to Dr. Now and be proud of. He is extremely nervous as he steps onto the scales,
22:33praying that he has lost the weight he was supposed to. But sadly, Juan has only managed to
22:37lose 17 pounds since his last visit to Dr. Now. I'm thinking he's not going to be too pleased with that
22:43result.
22:43So it looks like you did better this time than last time, huh?
22:48Yes. So what's going on with you?
22:52I'm not really sure. I made all of the changes after the nutritionist came and talked to us all.
22:59So I'm not really sure. I'm definitely not eating any carbs. I'm having lower portions,
23:06sizes. So I think I'm doing much better with that as well.
23:10And while it's true that he has made some progress, will that be enough for Dr. Now?
23:14Do you think you're still eating with the emotional issues?
23:19I think I do. Sometimes, occasionally. And that's what I need to work on as well.
23:28So you get emotional and upset. You eat, you feel better?
23:32I've always, you know, I would eat to feel better.
23:35Okay. So we'll set you up to see the therapies to help you with your emotional eating and figure
23:42that out. So that's going to be a goal that you need to work for. So you think you can do that?
23:48Yes, sir. I know you believe in me. I know my family believe in me. And I'm not going to let
23:54any of you down. So I know I can do it.
23:57Great. Juan, I'm proud of your progress. And I think you can do better.
24:02Dr. Now can see that Juan has been working hard,
24:04but he lets him know that he needs to push himself a little more to be able to get to
24:08where he has to be. I'm disappointed I'm not off to a better start.
24:13And I'm concerned about how much harder I can actually work.
24:16But I gotta keep trying and that's what I'm going to do. I'm buying into this program.
24:21And I gotta believe that's the only way to get the results I need. And to keep myself alive right now.
24:27This meeting with Dr. Now has been a reality check for Juan,
24:30but he doesn't know if he's going to be able to do what Dr. Now wants him to.
24:34I'm heading to see the therapist, Dr. Paradise, that Dr. Now wants me to see.
24:38And I'm feeling really nervous because I don't know how it's going to go.
24:43Part of it is just going to a new place. Because I don't know what kind of mobility challenges I
24:48might have to face. So I'm thankful I have my brother Bam here to help me,
24:51just in case it's difficult to get up the stairs or anything like that.
24:55After getting back home, Juan decides to go see a therapist to see what mental barriers
24:59are keeping him from letting go of food as his emotional crutch.

25:31I grew up with my mother and father. My parents divorced when I was around 11 years old.

26:14Thanks for coming in. The reason I brought you in is he was talking about being needy, and I noticed how helpful you were to him coming in, but I noticed that neither of you seem to like that.
26:28He knows, like, he knows I hate being needy, you know? And, you know, I have no one to blame for myself. I did this, you know?
26:36Does he want you to lose weight?
26:37Oh, yeah. Of course.

26:40I want to lose weight and get back to a mobile weight, a working weight for himself, where he can move around and be independent like he wants to be.
26:49If you had a more fit version of your brother, what would you want him to help you with, like, physically?
26:59It's been kind of one-sided, hasn't it? You've been getting a lot from them, and you haven't really been able to give much back.
27:07Oh, he can drive. He can drive more.

27:13My guess is you're even more mobile than you think you are.
27:16Dr. Paradise decides to bring his brother inside, and the two of them have a conversation about their relationship.
27:22I'd help hurting people, and at this state, it's impossible for me to help hurting people if I can't help myself.

28:11Quite honestly, it was a little embarrassing first because immediately some things that was built up began to come out.
28:21But it was a relief.
28:23I would say that I'm eager about driving, you know, my brother.
28:27And, you know, I'm going to take the challenge, and I'm excited about doing that.

28:37After the session, Juan feels like he knows himself a lot better, and he knows exactly what he needs to address to be able to move on with his life.

28:47It's been a couple of weeks since my appointment with Dr. Paradise, and I've been trying to do some of the stuff he told me to do.

29:07So it's been a bit of a tough sell.
29:09It's been such a long time since Juan washed the car. I'm very proud of him.

29:18And I'm just happy every time I see him doing more and more.

29:25So it's like his life is just coming back together like it should.
29:30And therapy has had an instant effect on Juan, who is now starting to get a lot more active,
29:35instead of just sitting around and waiting for food.
29:36I'm on my way to see Dr. Now for a follow-up visit, and I'm really excited about it.
29:42I think I've been doing pretty well. I've been following up with the plan.
29:47I've been working with the nutritionist and the therapist, and I think things are going well.
29:52I'm feeling optimistic because I've been working hard.
29:54And it feels like everything I'm doing is exactly what I'm supposed to be doing.
29:59My Dr. P homework hasn't been perfect, but I've been applying myself across the board.
30:04The next month, he is heading back to Houston again, and this is the appointment that makes or breaks the deal.
30:08It would mean a lot to me if it were enough.
30:12Because the weight loss surgery on top of this other change I'm doing would be a really valuable tool.
30:17I just know it. I was down to 601 two months ago, and I need to have dropped another 70 pounds.
30:23So if I did that, I'll be down to 531 or lower.
30:28And Juan is confident that his progress this time has been better than earlier.
30:32But do the scales agree?
30:35Your weight today is 577.
30:37Bigger weight loss than last time, so that's encouraging.
30:40But I don't know if Dr. Now will see it that way, and I'm afraid to find out.
