00:00I have insomnia really badly, so I don't sleep very well because of my size.
00:06I never really know if when I go to bed, if that's going to be the last time.
00:11So it's very hard to sleep when I have the different thoughts racing through my head.
00:17Karen Suffren is a 42-year-old from North Carolina who weighs over 640 pounds at the beginning of her weight loss journey.
00:24And she wants to get better for the sake of her children who have had to bear the brunt of her deteriorating health.
00:30My mother, if she needs something but she can't reach it and doesn't feel like getting up, she'll call me to help get it for her.
00:37Which, she apologizes, but I don't really mind because I'd rather her ask than do things on her own and get herself hurt.
00:45Her daughter Amber hates watching her mom live a life where she is completely dependent on her, but she knows that she has to be there for her through this hard time.
00:53I am asthmatic, so I do feel short of breath a lot during the day.
00:59Like, suddenly I feel like I can't breathe or my chest is tight.
01:02The fact that I'm winded just walking from my bedroom to the bathroom is not good.
01:08I'm supposed to take medicine for anxiety, but I haven't been to an appointment in a while, so no medications right now.
01:15It's hard to be compliant on medications when you don't go to your doctor's appointments.
01:18As of now, I am hastbound and cut off from the world.
01:21But her weight is just the beginning.
01:23Karen has a bunch of other physical and mental health conditions that she is battling as a byproduct of her obesity.
01:30My mother, she is a stress eater.
01:32I do think she uses food as a source of comfort, but it's just never occurred to me to have a problem with it.

01:44The best way to describe food addiction is just food is constantly on my mind.
01:50My meals contain mostly really high calorie, high fat things.
01:55It's painful when you're feeling full and you eat anyway.
01:59It's like you feel like your stomach is going to explode.
02:01Eating put me in the size body that I'm in that keeps me prisoner in my home.
02:06Oh man, everything looks so good.
02:07And the only thing that makes me feel better is when I eat.
02:11And the only way for her to deal with all of this stress is by eating the unhealthiest food imaginable.
02:16And once Karen starts eating, the only thing she can think about is how delicious the flavors are.
02:21As soon as she is done with a meal, she is immediately looking forward to the next.
02:24When I look back between my childhood and now, I do see where the first time I used food as a security blanket.
02:31There was at an age where I was given an assignment, I didn't understand it.
02:36And the person who was in charge of helping me with my homework, they beat me like really bad.
02:42I'll just never forget that feeling and wondering why.
02:44Because I guess they thought I was just being defiant, but you know, I didn't know what they wanted to do with me.
02:51It just made me feel like, you know, I didn't matter and that what I was feeling wasn't real.
02:57But here's the thing.
02:58Her emotional dependence on food stems from her childhood when Karen discovered that all her stress would fade away as soon as she started eating.
03:05My grandma and my mom had to work long hours at the hospital together.
03:10My grandma worked from the morning shift to the afternoon shift.
03:14And then my mom worked nights.
03:16So a majority of the family responsibilities just fell on me because I was the mature one.
03:19And it was a lot of responsibility, but, you know, what was the alternative?
03:24The problem is that because of all her childhood trauma, Karen still turns to food when things get tough.
03:30It's the only way she gets through the day.
03:32It made me feel like nothing, like no one, like being isolated and alone and feeling shame was just feelings I didn't want to talk to anybody else about.
03:40So I started self-harming, binging, and then throwing up.
03:44I remember the first time I did it, it was by accident that I did it.
03:47And then I realized it was something that helped.
03:51So I just kept doing it.
03:53Over the years, Karen's self-esteem has led her down a dark spiral and she doesn't see any way out of it.
03:58And the worst part is that she kept all of it to herself because she didn't want to bother anyone with her struggles.
04:03We got to, um, go to the grocery store.
04:06Even though I kept it hidden, I was still struggling with my own depression and anxiety.
04:12And then during one of our staff meetings, I had a panic attack.
04:15It was a really big one.
04:16So I did work from home.
04:18And being at home was less stressful.
04:21But I would eat and eat.
04:23I just started to put on more and more weight.
04:25And by the age of 23, I was nearly 400 pounds.
04:28The truth is that Karen has not had a very easy life and she knows that.
04:31But through all of it, the one thing that she has always been able to rely on is food because food never leaves her.
04:37Now that I'm the highest weight that I've ever been, even though I don't know what that is, I can just feel it because my mobility is bad.
04:45Seems like I lose more mobility every day.

04:52Try to find where there you have the snacks, like potashions and stuff, or snack cakes.

04:58The worse my anxiety gets, the more I eat, I eat until it physically hurts.
05:04And now my doctor says I've become agoraphobic.
05:07At this point, the only time she finds the energy to step out is when she has to go out and shop for groceries.
05:13But even that is a major challenge and she mostly sits in the car while her son does all the work.
05:18Okay, so what about the potato chip aisle?
05:20I hide a lot of stuff from my children because making sure my kids think I'm okay has always been important to me.
05:28So I try to be open with them and honest.
05:31It hurts.
05:32I can't go anywhere comfortably and I don't want to, you know, embarrass my children, even though they would never say that.
05:39That's how I think.
05:40And I just, I just want to protect them.
05:43So I just decided to stay to myself so I'm not inconveniencing anyone.
05:47So I just stay in my little bubble.
05:50Karen's shopping consists of her favorite unhealthy, greasy, and sugary snacks because that's all she eats.

05:58You know, actually, when you were down the cereal aisle, I should have had you get some cereal.
06:03You want your fruit juice?
06:05Yeah, fruit juice.
06:06Perfect.
06:07Get the apple and the orange, peach, mango.

06:12I forgot to have you give me some chocolate milk because I have those cookies.

06:18My children, they do try to make things easier for me.
06:21And she is embarrassed by how unhealthy her lifestyle is, so she hides certain things from her kids.
06:26When I started growing up and I really looked at her more as a person instead of just like my mom, I see she has some self-esteem issues and I try to like, you know, just be there for her.
06:37I do, I live for my children.
06:40I promised them that I wanted to be there for their graduation, so I'm definitely getting healthy for them.
06:45I want to make the changes that I need to make.
06:48For now, Karen's children do everything she asks of them because they know how hard life is for her.
06:53But in doing so, they are enabling her unhealthy eating habits, which prevent her from ever getting better.
06:58Now, I'm going to get these dishes and everything put away, just straighten up so I can get ready for my appointment with Dr. Now.

07:12Karen is now ready to go to Houston to meet Dr. Now, the only person who can help her.
07:17I desperately wanted to go see Dr. Now, so I called this clinic and I set that up.
07:21But a couple of days before going to Houston, I started to have panic attacks and told them I couldn't come.
07:28So today, I'm doing a video call with Dr. Now.
07:31However, going there is not easy for Karen because of her anxiety, which is why she decides to meet him in person.
07:37So, you were supposed to come down and see us.
07:40So, what happened?
07:41I don't, I don't, actually, I don't even know what it is. I just know, like, I couldn't do it.
07:45Like, I became agoraphobic.
07:47Like, a couple years ago, I started going out less and less and then it got to the point where I just stopped going out at all.
07:54Even now, it's to the point where it's too much and I can't go anywhere.
07:57Like, when I was thinking about going to see you, I got dizzy.
08:02I felt like I couldn't breathe with just thinking about leaving to travel.
08:06So, that's what happened.
08:07And I was like, oh my God, I can't do this.
08:09Safe to say, Dr. Now is shocked to see how bad Karen's condition is and he knows that she needs to start taking responsibility for her actions instead of acting helpless.
08:18The last time I got on a scale, I was 6'20", but it's been a while. So, it's probably more than that.
08:26At what age you noticed you had weight problems?
08:29I've always had weight problems. Like, when I was in ninth grade, I was like 300 pounds. So.
08:37What is the goal weight you want to be at?
08:38Um, I don't, I don't have like a goal weight in mind. I just want to be healthy.
08:47So, your biggest issue not being out of house because people judge you to size and you worry about that?
08:55I don't, I don't, actually, I don't even know what it is. I just know, like, I just feel like I'm just a hassle for people.
09:01I'm inconveniencing them and I'm giving, I'm making things harder than it should be.
09:04Karen keeps giving the doctor's excuses about why she was not able to make it to Houston, but doctor now knows that it's a way for her to avoid addressing her weight problem.
09:12For lunch, I'll either order something out and then, like, dinner I'll cook or on weekends, typically after work, I'll actually order out dinner too.
09:20So, there's times where I'll order out for lunch and dinner.
09:23You understand, your weight is related to what you eat.
09:27Right, right.
09:27And if you don't lose weight, you eat less.
09:29Right.
09:30If you don't know what to eat, what not to eat, at least you understand eat less is going to make you lose weight.
09:36We can give you the instruction and roadmap of what to do, what not to do, but this is all going to be up to you to make those changes and stick with healthy eating habits.
09:47Because I want you to lose 50 pounds over the next two months to show me that you're serious.
09:52It's easy to see that Karen has practically forced her children to enable her eating habits, and she needs to stop putting her kids in a position like this.
10:00Doctor now is absolutely shocked at how unserious her attitude is.
10:04So, do you think you're going to be willing to work with a therapist and see what issues are driving you to become a cruise?
10:13Yes.

10:18I can drive, I just don't, but my son also drives.
10:22Okay, so we can set up a clinic at your neighborhood to go to and check your weight so we can monitor your progress and see where we are.
10:33Because if you don't do that, moving too soon is going to be a problem for you.
10:38Weight loss surgery cannot become an option.

10:40The doctor gives Karen a goal that she needs to hit if she wants to qualify for weight loss surgery, but he makes it clear that it's up to her to ensure that she moves towards getting better if she wants to live a long life.
10:52Karen also has to head over to a local clinic to check her weight, which is something she hasn't done in a long time, along with getting help for all the mental struggles that have kept her from losing weight.
11:01It's been four days since I talked to Dr. Now.
11:04I got his book in the mail and the papers and went over them with him.
11:09They're really strict, but I've been starting them.

11:29After her conversation with Dr. Now, Karen is trying to stick to the diet he wants her to follow, and of course, changing up her eating habits has been anything but easy.

12:16Some of these exercises, they're simple, but I mean, I already started sweating, so I'm just going to do these a couple times a day and just follow Dr. Now's plan.

12:35You have to give it to Karen for giving this thing her best shot, despite the fact that it's not easy for her body to start moving in ways that it hasn't for years now.

13:01But for some reason, her daughter is not in favor of Karen getting a bypass surgery because of how dangerous it is.
13:08I'm still tired from yesterday, and I'm really nervous about talking to the therapist, because I'm embarrassed about my relationship with food.
13:17But I'm hoping it's going to help me start to work up the courage to go out and do what I need to get healthy.
13:24It's easy to see that Karen is torn between a bunch of emotions, and all of that affects her ability to keep pushing with all her energy.
13:31And as she sits down to speak to her therapist, she is as nervous as ever.

13:39What were you like as a kid?
13:41I mean, I was the responsible one, so even though I had, like, two older sisters, I was, like, you know, the mom.
13:46While my mom was working, you know.
13:48Are you that kind of person?
13:49Do you tend to be the one taking care of other people?
13:52I mean, well, it's what I do for a living and what I'm going to school for, because I do like to, you know, take care of others, you know.

13:59So we're on to something that's a theme.
14:01Thankfully, Karen finds it pretty easy to open up to Dr. Paradise, who helps her figure out where her addiction to food actually stems from, and how she can start working on getting rid of it.
14:11How'd you end up in the hospital?
14:13I just, I don't know.
14:14I was just, I was going through, like, a lot, like, mentally, and, like, I really, I always had really bad anxiety and depression, too, for sure.
14:23Did you get put in the hospital because you were thinking of hurting yourself?
14:26I tried to, so, yeah.
14:29So you made an attempt on your life?
14:31Oh, yeah.
14:31You know, I'm also noticing, um, you're talking about some pretty heavy stuff with pretty, you know, pretty intellectual, even-tempered.
14:41It's easy to see that Karen has a lot of emotions that she has buried deep down inside, and opening up about them is a struggle for her.
14:47But she knows that she needs to be honest about what she thinks and feels if she ever wants to overcome her trauma.
14:52I'm going to give you a homework you're going to hate.

15:30It's easy for him to see that Karen's issues are deep-seated and that they can't go away in a single day.

15:42So Dr. Paradise asked me to open my curtains and make sure that I go outside and that I walk the dog.
15:51It was hard because I don't like so much exposure.
15:54But that's something that I could probably implement daily.

16:02It's the second month of Karen's weight loss journey, and she is trying to do what Dr. Paradise has asked her to.

19:55Safe to say that Karen ended up not going to Houston for her appointment,
19:59and it's evident that she has not made any real progress with her weight loss.
20:02But we will never know that either because she has not gotten herself weighed.
20:07I'm scared of that since she missed your appointment and she didn't respond to her phone for the past few weeks.

20:40I've been following the plan, the eating plan, and I've been exercising every day.

23:06Turns out that Karen has lost some weight since the beginning of her journey, but she was hoping to have lost more.
23:11I wish I would have gotten a little bit more credit for the hard work that I put in.
23:15I didn't really have my starting weight.
23:17I just feel that it was, I think it was in the high 600s, even maybe close to 700.
23:21So, because I do feel I lost at least almost 100 pounds because I can definitely feel the difference in my body.
23:27But I'm going to continue to just work with the program and follow Dr. Now's diet and exercise so I can prove myself to him and show him that I'm going to be able to do this.
23:40But at the same time, Dr. Now wants to see if she can keep the momentum going and lose more weight in the following months.

25:05And I feel like I've made great progress with my weight loss and stamina.

25:12But Dr. Nath told me I'm close to lose at least 50 pounds.

26:54Finally, though, she makes it to the clinic, but when it comes to getting out of the car, she just doesn't want to do it.

27:12After driving all the way there, Karen immediately wants to go back, and that's exactly what she does, despite Amber motivating her to go in.
27:19It's been about a week since I was supposed to have my appointment at the clinic.
27:24This time, I didn't want to avoid Dr. Now.
27:27He agreed to send the scale to me to see if I made progress.
27:31I told him he'd be really happy with how much I lost, and I'm hoping that convinces him how hard I'm working.
27:37He said whether any of this continues would come down to whether I made any real progress or not.
27:42And I'm looking forward to being able to show him that I have.
27:45Last time, I was at 604 pounds, and I was supposed to lose at least 50 pounds, and I think I lost somewhere close to 200 pounds.
27:55And so I think I'm going to be in the 400s today.
27:58And you have to appreciate Dr. Now's commitment, because once again, he is sending a doctor over to Karen's house just so she can get her weight checked.

28:13I'm kind of frustrated right now because I weighed myself this morning.
28:17The scale is not going down to the number that I think it should be at because I know, you know, I've lost 100 pounds since starting the program, and it's just not showing that.
28:27So, and I know you're not going to be happy with the numbers, so I'm just kind of frustrated and processing in that.
28:32I know that can't be accurate, so I think the scale is off.
28:35Well, the scale most likely is correct, but sometimes moving around the scale may change the zero balance, so we may have to redo the zero on the scale one more time.

29:04While it's true that Karen has lost weight, she is only 20 pounds lighter compared to what she thought she lost.
29:10But she seems to think that there's something wrong with the scale because she is sure that she lost 200 pounds and Dr. Now is in disbelief of her delusion.
29:18So what is the scale saying? I can't see it.
29:24It said 585 this time. That's not right, Dr. Now, because it's higher than before, and that's not right.
29:30Karen, this is a professional scale. It's not your everyday scale. They are precise and is accurate.
29:38So you may think you lost more weight, but in reality, you haven't lost as much as you think you have.

29:47But Dr. Now makes sure that Karen gets her weight checked once again just so she can be proven wrong.
29:52So you haven't shown me that you're going to be able to come to Houston even if we approve you for weight loss surgery.

30:21I'm going to lose more weight and be at the goal that you want, and I'm going to go to the local clinic.
30:26And while Karen swears that she feels 200 pounds lighter, Dr. Now gives her a major reality check to bring her back to Earth.
30:32It's probably unlikely that the scale is going to be inaccurate and maybe about a pound or two difference, but it shouldn't be too, too off.
30:41It's just frustrating because I know I'm doing really well.
30:44I feel that the scale wasn't accurate because the nurse had a hard time zeroing out the weights.
30:51I just want to prove to Dr. Now that I'm going to lose the goal that he gave me to lose so I can show him that I'm serious about it.
30:58But despite all this, he is willing to give Karen another chance, even though she clearly doesn't deserve it.
31:03But at this point, everyone knows that Karen is not going to be able to make any progress unless she starts taking responsibility for her actions.
31:10I've worked hard these past two months, and I know I've made more progress, but I've decided I'm not going to put myself through everything it takes to try to go out again if I'm just going to be told it's not enough.
31:26So I call Dr. Now to ask if the scale could be brought to me again, and if not, then I understand.
31:32But I won't be pursuing his program anymore if that's the case.
31:35It's the seventh month of Karen's journey, and she still claims that she is making progress, despite the fact that things don't look any different for her.

32:37I have mixed feelings about what's happening between me and Dr. Now because I'm grateful to him and his program for giving me the motivation to start making the changes that I need to lose weight.
32:47But I am not happy with the way he and his staff are treating me.
32:51Sadly, Karen isn't even close to the goal Dr. Now set for her, and once again, she thinks there is something wrong with the scales.

33:06It's because I'm working my hardest, Dr. Now, and I don't feel like what the scale is saying is accurate, and I don't feel like there's any point in talking to you about it.
