00:00I feel like the life that I am living right now is a nightmare and it's because of how bad my body has gotten.

00:36But it's so bad that I have to bother my grandparents by living in their cabin.
00:41Life is anything but easy for Jackie and she knows that unless she loses her weight, things are always going to be this way.
00:47After I'm cleaned and dressed, that's usually when my Aunt Teresa comes in to check on me.

00:59My Aunt Teresa, she is like a second mother to me.
01:03She always wanted a girl.
01:06She has three boys.
01:08She does everything she can for me.
01:10She helps me around here.
01:12Me and my Aunt have a really good relationship.
01:15And besides my grandparents, she's the only person that I see regularly.
01:20All right, Jackie.
01:21It's hard for Jackie's grandparents to watch her struggle so much, but they know that they can't do anything unless Jackie wants to get better herself.
01:28Growing up, there was no reason to think I was not going to be happy.
01:32My uncle, like everybody loved his gumball because we had so many kids in the family at the time.
01:38And to this day, the taste and smell of certain foods.
01:41I don't know, it just makes you feel whole.
01:44But my mom was an alcoholic.
01:47It would alter her because she would get more aggressive.
01:51She would always come at me with, like, the belly.
01:54And the worst part is that Jackie's addiction to food runs deep.
01:57In fact, it goes all the way back to her childhood.
02:00At school, I was always taller than my teachers.
02:02So I always stuck out like a sore thumb.
02:06People made jokes about me.
02:07I just wanted to have the same life as everybody else.
02:10But at eight years old, I was already 130 pounds.
02:14And I had a hard time making friends because my weight was getting worse.
02:18I didn't know how to stop myself from eating.
02:21So I just kept to myself all the time.
02:23Because food made me happier than people could.
02:25And by the time I was in high school, I was at 565 pounds.
02:30Jackie's parents weren't the most involved in her life.
02:32And as a child, she had to learn to fend for herself.
02:35And in the middle of all of this, she relied on food for comfort.
02:37Even with how much I was eating to cope,
02:40and even though I couldn't really stand being on campus,
02:44somehow I was able to graduate.
02:46I didn't want to go to college because I was over 600 pounds.
02:49And I was worried about college campuses not being accommodating about stuff like that.
02:54Getting a job at my size was hard.
02:56But I eventually was accepted and got hired on.
03:00At a grocery super center.
03:01And then I realized that was really hard being always on my feet.
03:05And then little kids looking at you, they would point and laugh.
03:10I realized that was too much.
03:12It was too much stress.
03:13Eventually I got hired on for doing customer service.
03:17It was around that time when I got way up into the 600s.
03:20Through all the trauma, food has been the one thing that Jackie can fall back on
03:24to deal with her emotions.
03:26It was not long after that until my dad stopped taking care of himself.
03:31One day I came home, opened the door, and saw him on the ground dead.
03:35And that's when I felt the most alone I ever had.
03:39Just me, myself, knowing that everything that I've known is gone.
03:44I couldn't take care of the house.
03:45That's when I talked to my Aunt Trace and talked to my Grandpa.
03:49And they helped move me into this cabin here.
03:52I think I'm getting full.

03:56Jackie's past has been extremely traumatic, which is why she has developed an unhealthy
04:00emotional dependence on food.
04:02And that has been hard to get over.
04:05When I get out of breath, when I fall, it's just a lot on my mind going places right now.
04:12I feel judged by the outside world all the time.
04:14Whenever Jackie steps out of the house, she realizes just how bad things are because it's
04:19hard for her to take a few steps without losing her breath.

04:32But the minute she sees all the food in the aisles, she loses all train of thought.
04:36And the only thing that matters is that she gets all her favorite junk food as fast as she can.

04:44I do feel that I'm addicted to Mexican food.
04:47Tacos bring back memories of my dad being together.

04:52It just brings back memories of the family being together as a whole.
04:59Feet doing okay after all that walking.
05:01She has been in the hospital at least twice, I believe, with cellulitis from the lymph leaking
05:10out of her legs and then setting up infections, and that's kind of scary.
05:16But at this point, her grandparents are also enabling her unhealthy eating habits, but that's because they don't know how else to make their granddaughter happy.

05:32I decided that I need to get help.
05:35So today, I'm driving to Houston to meet with Dr. Now, a bariatric surgeon who helps people my size.
05:42It's going to be about a five-hour drive, and that's the longest that I've been in the car since I've been this big.
05:49However, she decides to take some action for the sake of her grandparents and decides to go to Houston to meet Dr. Now.
05:56You all right, Jackie?
05:57Yeah, just rearranging.
06:00How'd you sleep last night?
06:02My back was hurting for a little bit, so I had to get up and move around for that.
06:08But once you lose your weight, well, you'll feel better about getting around and getting out and doing things like you used to do, Jackie.
06:19Of course, the long drive is anything but easy for Jackie, whose body just isn't strong enough to take all the stress.
06:25I've been on the road for a couple hours, and it's been pretty uncomfortable with the way that my stomach is hanging.
06:31There is a good taco place up here.
06:33So we're going to make a pit stop and get something to eat.
06:36It's too painful and too humiliating for me to go in.
06:40Eating like this is probably not something Dr. Now would want me to do, but it's going to make me feel better.
06:50Oh, yeah.
06:52I believe I can survive these chicken wings.
06:55They smell good.
06:57This is really good.
06:59And just as soon as things start getting hard, she immediately wants to turn to food.

07:04All the tall buildings.

07:08I'm starting to get scared of what will happen if he turns me away.
07:13Maybe I'm just too big or too far gone.
07:16Man, we did good, didn't we?
07:18While that gives Jackie the boost of energy and motivation she needed to keep going,
07:22it's evident how much she relies on food to get through the day.
07:25I'm worried to step on the scale, especially in front of my grandfather.
07:29It's embarrassing, and I feel like I'm letting him down.
07:33The last time I was weighed, I was in the hospital for kidney stones.
07:37I was right at 700, and I was trying to eat less since then.

07:43Because I hated seeing that number.
07:47When she gets to Houston, she's nervous about what Dr. Now will have to say.
07:50And as Jackie steps onto the scales, she only hopes that she is not too far gone to receive help.

08:04Because I feel like a disgrace to the family.
08:07But mostly I'm scared because how high my weight is.
08:10Because if I'm too big to get Dr. Now's help, I'll be totally helpless.
08:15It's been a while since Jackie has been weighed, and she is shocked to see that she weighs so much at this point.
08:19So, what have you done to lose weight?
08:22I've been going to a gym, but I stopped going because I fell, and then I embarrassed myself there.
08:27Okay.
08:28When you're 700 pounds, exercise is not a factor in your weight loss.
08:32The only factor that affects your weight loss is your food.
08:36So, how many times do you eat in 24 hours?
08:4024 hours, probably like once or twice.
08:43Do you snack in between?
08:46Not really, because I'm working.
08:48I do work from home.
08:49Because it's just been on my body for so long.
08:51It's not, it's hard to come off.
08:54Well, you know, your weight is related to the calorie you're taking in one day and calorie you're spending in one day.
09:01And what we're going to do is give you some diet and some exercise plan.
09:07The diet will be a 1,200 calorie a day diet, high protein, low carb.
09:11And if you follow that, you should be able to lose easily 100 pounds in one month.
09:18Dr. Now is shocked to see how bad things have gotten for Jackie, especially with all the hanging fat on her body.
09:23The goal I'm going to give you is to lose 70 pounds in two months.
09:27Okay.
09:28So, if you follow those, you're going to be able to do that.
09:31You think you can do that?
09:33I definitely can.
09:34Okay.
09:34I have a great support system with my grandparents and my aunt.
09:38You're encouraged to eat less?
09:40Yes.
09:41I told her I didn't want her dying before I did.
09:45So, you have no enablers except yourself?
09:48And I realize that now.
09:50Okay, good.
09:51And 210 is the weight that you should concentrate.
09:56So, this is 500 pounds that you have to lose?
10:00While Jackie claims that she has tried to lose weight, Dr. Now makes her realize that it's just not enough and that she has to work a lot harder.
10:06Hopefully, going over the diet and explaining that to Jackie will help her start to focus on changing her eating habits.
10:15But we'll see if she starts to move in the right direction.
10:20And the only reason that she's still able to function at that weight is because she's still young.
10:27And that factor is changing every day.
10:30Jackie is not far off from reaching 800, 900, and 1,000 pounds.
10:37And once she gets to that point, she'll be better than and she's not going to make it.
10:42The doctor can see that Jackie has a great support system and he hopes that they will give her the push she needs to keep going and making progress because time really is of the essence with her condition.
10:52I'm glad to have Dr. Now's guidance.
10:55What he wants me to do is a lot, but I think I'm ready for it.
10:58I know I have to change and do what Dr. Now says before it gets too late for me.
11:04So I'm going to do everything I can to turn my life around starting now.
11:08As she walks out of her appointment, Jackie is nervous about all the changes she has to make to her life.
11:13But she is ready to commit to Dr. Now's plan and become a healthier version of herself.
11:18Right now, I'm still trying to get used to living by myself, being an adult, I guess you would say.
11:23It's been a few weeks since I've seen Dr. Now and started his program, and it's been a bit of an adjustment.
11:30The food that I've been eating has been making me feel a lot better.
11:33Mostly for breakfast, I've been having like a yogurt or like a pudding.
11:36Then for lunch, I've been doing like a salad.
11:39I pre-made salads for the whole week.
11:41And then for dinner, I do like chicken.
11:43So I've been feeling a lot more energized.
11:45I'm pretty busy with work every day.
11:48But now I have to take breaks to get my exercise in.
11:51To make sure I get enough activity to fill up the hour in the morning.
11:55Doing the exercises, it's kind of hard to deal with the stomach hanging down so low.
11:59But it's a work in progress.
12:02In the second month of her journey, Jackie has had to turn her eating habits around completely.
12:06But she has been feeling better and much more energetic.
12:09Having her and my grandfather to support me has been good.
12:12All right, girl.
12:15Losing 70 pounds in two months seems like a lot.
12:19But I'm giving it my all to do it because I want the surgery.

12:28I normally walk on the road.
12:31Ah.
12:32Jackie is really starting to enjoy the quality time she spends with her family out on walks
12:36or while she works out.
12:37And she hopes that by the end of the month, she hits the goal Dr. Now has set for her.
12:41I'm hoping to see good results today.
12:45I feel like I've lost a fair amount.
12:47I feel like my stomach area is like up here.
12:50It feels like it's getting better.
12:51I'm back in Houston with my Aunt Teresa.
12:54And we're headed to Dr. Now so I can get weighed and see if I made it to my weight loss goal or not.
12:59A few days later, it's finally time for the moment of truth.
13:02And Jackie is feeling pretty hopeful.
13:06Please step on the scale.
13:07Jackie is nervous as she steps onto the scales because everything depends on the number on the scales.
13:18Your weight today is 689.
13:20I'm a little upset that I didn't lose more because I thought I was doing better than that.
13:25So I'm not sure what happened.
13:27And I don't want them to think I don't care about this because I do care.
13:31And I just don't know why my progress wasn't better.
13:34While Jackie was supposed to lose 70 pounds and hit 640, she has only managed to lose 20 pounds, which is still progress but just not enough.
13:41What's going on with you?
13:43This is a really slow start for you.
13:46You need to pick up the pace.
13:48Because you only lost 20 pounds in two months, which is nothing for someone at your size.
13:54When you're 700 pounds, losing 20 pounds should be something you lose in one week.
14:01So what do you think you changed what you're eating company to try to make the progress?
14:06We've been making progress by sorting meats out into the four ounces, like the book says.
14:11And then we've been doing salad preparations, like putting it, like a cup of salad and then some, like a third cup of kale.
14:20Well, if you're doing that, you should have easily lost over 100 pounds in one month.
14:26Of course, Dr. Now is not happy about where things are, considering that he gave Jackie a lot of time to hit her goal.
14:33If you're 700 pounds, you have to eat 7,000 calories to maintain that weight.
14:39If you're 200 pounds, you eat 2,000 calories to maintain that.
14:43So it's a proportion to your body weight.
14:45So if you're heavier, you can lose weight much faster.
14:50So Jackie, cut back on your eating and work to lose at least 35 pounds a month.
14:56He gives Jackie a reality check and tells her that she can cut down her diet way more than she has because she is still eating more than the average person.
15:03Jackie is clearly still overeating despite what she thinks she's doing.
15:09All we can do is give her some tools and a roadmap and hope she chooses to start following it before it's too late.
15:19But Dr. Now is not too sure whether his words got through to Jackie because she seems to be ignoring all the advice that he gives her.
15:25I wish I had better progress today.
15:28I'm discouraged, but I'm going to try to do better next time.
15:31I guess I'll just cut back more like Dr. Now said and see if that's enough.
15:35Because I really do want to do better, so I'm going to keep trying.
15:40Jackie is not happy as she walks out of the clinic, but she is determined to follow Dr. Now's advice and cut back on food even more.
15:46Even though she has no idea how she will be able to do it, but she knows she has to try.
15:51But now I feel less sore.
15:53So that tells me that I'm making progress and that my body is getting to a better place.
15:58Dr. Now has been wanting me to eat less than what I was before, so even cut more out.
16:03I'm basically not eating breakfast or maybe eating a yogurt for breakfast and doing the salads for lunch when I'm working and then doing a meal after I get out of work.
16:13In the fourth month, Jackie has stepped up her game and is focusing a lot of energy on working out while following Dr. Now's diet.
16:20Yeah, I took up some pork chops earlier.
16:22Oh, okay.
16:23Well, I'll keep you company while you do that.

16:47I really want to lose more this time before I go back to Dr. Now in a month because I want to get healthy.

16:55This looks a lot healthier than what you were eating.

17:03Her family is playing a huge role in helping her manage her diet and making sure that she doesn't slip back into her old ways.

19:20But at least you lost 40 pounds so far.

19:25While the doctor appreciates Jackie for losing the weight that she has,
19:28he wants her to know that at her age, she should be easily hitting the goals he is setting for her.
19:32Unfortunately, Jackie didn't improve her progress like she needed.
19:37I'm glad she at least losing some weight consistently.
19:42But 10 pounds a month will take her almost four years to lose what she needs.
19:47So she needs to work harder or it's going to be clear soon
19:51that there is not much more we can do to help her.
19:55The doctor gives Jackie two more months to hit her goal.
19:58If she does that, he will approve her for weight loss surgery.

20:03I feel like I'm letting everyone down and that is a really bad feeling.

20:27The last time I went to the gym was, I think, a week or two ago that Grandpa William came with me.
20:33Oh, God.
20:35Lately, I've been having a tough time.
20:37Last month was the anniversary of my dad's death.
20:41And that was hard for me.
20:42So I gave in and ate some stuff I shouldn't have eaten.
20:46Because I was just sad and didn't know what else to do.
20:49I know I messed up.
20:51And I've been trying to make up for it since then by working out extra hard.

20:58And in month six, she is ready to start making some major changes.

21:48And I can feel how sore my body is.
21:51So I know it is helping make up for how I ate over the last month.
21:56The last time I was here two months ago, I weighed 669 pounds.
22:00And the goal was the same, to lose 70 pounds.
22:03So if I did that, I'd be right at 600 or 599 pounds.
22:09And I really hope I made it there.
22:11The next month, it's time for Jackie to face the music again.
22:14And she knows that if she fails this time, it's over.

22:39The scale was showing that I gained weight instead of lost it.
22:43Yes, I can see that.
22:44I think you gained 17 pounds.
22:46Why?
22:47Just with depression going on.
22:50And then my dad's death date just occurred.
22:53So that was really stressful to go through.
22:56So what do you tell me that you got depressed and you used that to justify eating what you
23:04want to feel better?
23:06Do you want to change your life or do you want to kill yourself with the food?
23:10Yes, I want to change my life.
23:11So what is your objective coming here and telling me you gained 17 pounds because you
23:15were depressed?
23:16But sadly, Jackie has actually put on weight instead of losing it and that completely derails
23:20her progress.
23:21And of course, Dr. Now is shocked at the sudden turn of events.
23:24I am going and getting burgers and stopping at other restaurants.
23:30So what did you eat at home?
23:32Like the salad that I had.
23:34I make pre-made salads.
23:36I had that at home.
23:37But you told me you should have lost 30 pounds, but you haven't.
23:40I was just eating the wrong things, making the wrong things at home.
23:44Like what?
23:45Like pasta.
23:46So now the salad is turning to pasta.
23:49I understand that, but I don't eat constantly like you're suggesting.
23:53I don't do that.
23:53I mean, I may eat bad, but I don't eat like...
23:56You're just not telling me the truth to what you're eating.
23:59You're just not coming forth and telling me exactly what happened and what you did.
24:04You're the one that you need to be in driver's seat and be changing or not.
24:11Nobody else can do that.
24:13So at this point, we get you to see some therapists and see if you can make any changes.
24:18While Jackie claims that her sudden weight gain has been because of all the emotional stress she has experienced in recent months,
24:24he wants her to know that this cannot be an excuse every single time.
24:28At this point, Jackie has no excuse for all of her poor progress.
24:34She had all the tools to lose more than the goal I gave her.
24:38Maybe therapy or the fact that this is her last chance will wake her up.
24:43But if something doesn't, and Jackie comes back next time with little or no progress,
24:51then we won't be continuing with her.
24:54I don't want to lose my last chance, so I have to figure this out.
24:58And it's probably overdue that I talk to a therapist.
25:02So I'm going to do it all.
25:03But because Dr. Now can see that Jackie's mental trauma is a barrier to her weight loss,
25:08he sets her up with a therapist and gives her one more month to show him some progress.
25:13If she doesn't manage to do that, it's over.
25:15Jackie also thinks that at this point, it's time for her to start seeking help for her mental health.
25:19Today, I'm going to meet with a therapist that Dr. Now set me up with.
25:24And I'm hoping he will be able to help me get in touch with myself and see what is holding me back.
25:29Because I really want to move forward.
25:33And I'm getting to the point where I'll try anything.
25:35And once she gets back home, she wastes no time before setting up an appointment and going to meet Dr. Paradise.
25:40What was your childhood like?
25:42It was very traumatic sometimes because my dad, he was 6'8", so he stood very tall and then he got very angry.

25:53Did he have a temper?
25:54He had really bad anger issues.
25:56I remember CPS being involved.
25:58He would throw things, punch holes in the wall.
26:02Sometimes he hit my mom.

26:06But I'm guessing we're just scratching the surface.

26:09For the first time in forever, Jackie has someone to open up to about everything that she has been through.
26:14And she had no idea that all of this was weighing so heavily on her.
26:17That's a big deal, too.
26:18And then I found my dad on the floor whenever I came home.
26:22So I found his behind me.
26:22When he passed away, how did your dad die?
26:25We think it was due to his pacemaker.
26:27But again, you found the body.
26:31I'm so sorry that that happened to you.
26:34I still don't know what to do.
26:36I don't know how to move forward.

26:41I see, you know, the emotion's there, but are you protecting me from the emotion or are you protecting you from the emotion?

26:54The doctor realizes that Jackie has experienced major trauma, especially having been the one to find the bodies of her dead mother and father, and she confesses that it still haunts her to this day.

27:36I know I'm facing my last chance with Dr. Now, and that scares me.
27:41But I also have hope because my session with Dr. Paradise was really positive.
27:46And I feel like he pointed me in the right direction.
27:50So I went ahead and got myself a journal to write in.
27:53I tried stuff like this in the past, and I've never gotten very far with it.
27:58But I'm hopeful that it makes a difference.

28:18After therapy, Jackie feels like she finally knows what to do and how to manage her food addiction, and she hopes that it works.

28:58But it's clear that she does not want the past to hold her back anymore.
29:01I'm back at Dr. Now's, and this is the biggest moment yet in my weight loss journey.
29:06Because if I didn't lose 70 pounds this time, then that's it for me.
29:11And if that's it for me, then I don't know what I'm going to do.

29:16I was 686 the last time, so that means I have to be 616 pounds or lower today.
29:26The next month, Jackie is back for her weigh-in, and she is hoping for a miracle here.
29:31Because if she hasn't lost enough weight, she will no longer be part of Dr. Now's weight loss program.

30:03Um, it helped me get past some of my childhood issues that I had, the trauma that I had with my parents.

30:14And while she hasn't lost as much weight as Dr. Now wanted her to, she has still made significant progress.

30:20So with your current progress, we're going to improve weight loss surgery.
30:24But I'm going to schedule your surgery for two months from now, and I want you to get under 600 pounds.

30:33With the support that I have, I know I can do it.
30:35I think this being her last chance was probably more the motivation for her to do better than therapy was at this time.
30:44But she clearly has emotional issues driving her to eat.
30:48So therapy is going to be an important part for her long-term success.

30:55Dr. Now is happy to see Jackie's commitment to her health, and he approves her for weight loss surgery after two months, in which he asks her to lose more weight.

31:20This is a really big surgery, so I am nervous on that aspect, but I'm more excited about the future than I am nervous right now.
31:28In month 11, Jackie is finally getting ready to receive her weight loss surgery, and while she is nervous, she also knows that this is the beginning of a new life for her.
31:36Today, we are attempting to perform a GAS-60 on Jackie.
31:42She continued to make decent progress over the last few months, and since her last appointment, she lost another 25 pounds.
31:51And that was enough to show me that she's still trying to work hard, so we will move ahead with her.
31:57This operation will help Jackie to stay on track and continue addressing the things she needs.
32:04Okay, let's begin.
32:05But the procedure is risky, and Dr. Nao has to be extremely careful as he performs the GAS-60 on Jackie.
32:12A very big, long stomach she had.
32:16Jackie's surgery went very well, and we were able to receive GAS-60 on her.
32:21She's a very tall and long stomach, but everything went that good.
32:29Now we've got to go see, recover her and see how she's doing.
32:32The paniculis caused us the most issues, but we managed to work around that.
32:38Over the next month, she will be on liquid diet, and she will be able to lose another 35 to 40 pounds.
32:46And then after that, she should be able to lose 15 to 20 pounds every month.
32:52But we will keep an eye on things with her as we move forward to make sure that she doesn't get comfortable and start to slip back into her own habits.
33:03Thankfully, the procedure goes smoothly, and Dr. Nao sends Jackie into recovery.
33:06You're waking up, and later on we get you up walking, and get you moving around.
33:14But everything went very well.

33:21But now it's up to Jackie to continue the momentum and make sure that she doesn't fall back into her old patterns.
33:27It's been about a month since my surgery, and I'm doing really well according to Dr. Nao.
33:32I'm even a little bit ahead of the curve for how much weight I've lost last month.
33:37And Dr. Nao says that's good, because my surgery weight was a little bit higher than what he usually sees.
33:43But I lost another 41 pounds since my surgery, and I'm really excited about that.
33:49I didn't much care for the liquid diet, but it worked, and now I'm back on solid foods.
33:55Can't really eat much anyway.
33:57Since the surgery, Jackie has lost 154 pounds, and she continues to lose more every day.
34:03Not since you were a little bitty girl.
34:05It's the last time we were out looking at animals together.
34:10I am so thrilled that Jackie is able to get out and do things that make her happy.
34:16She's always loved animals, and I knew that this was something that she would like.
34:22If you're in the house all the time, you're missing out on things like this.

34:39She has made a wonderful recovery and is off the liquid diet,
