33:24when I found out that his nephew was the first officer
33:30I suggested that perhaps they should consider
33:34removing Captain Fernandez from the investigation
33:37because of emotional involvement and what have you
33:40the American investigators concerns soon vanished
33:44he was very objective
33:46I would say an excellent investigator considering that it was
33:49and not a distant nephew
33:51I mean it was his very close relative
33:54he did an outstanding job
33:58the black box in the Boeing 757 can emit a locator beacon for 30 days before batteries run dead
34:10the US Navy provided underwater remote operated vehicles to survey the debris field
34:16seeking the black boxes
34:21the wreckage confirmed that the plane went down in one piece
34:29I've done in-flight breakups that were spread over 15 16 miles and maybe a mile and a half wide
34:36which tells you instantly that the just what you know of the looking at the wreckage that
34:42this thing didn't hit in one piece it clearly was disintegrating as it was crashing
34:47but in this case it was a fairly tight debris field
34:51and obviously it hit at high speed and was fairly closely knit wreckage pattern
34:59the data recorders were retrieved from the 757
35:04brought to the surface the boxes were placed in coolers full of fresh water to keep them from oxidizing
35:10they were taken back to Washington for analysis at the NTSB
35:21the cockpit recorder could offer the evidence investigators sought
35:25every word spoken by pilot Schreiber and Fernandez
35:29and every unnerving alarm was recorded on audio tape
35:33the recorded voices were faint
35:40sometimes hard to make out
35:42but the chaos in the cockpit rang through with chilling clarity
35:46the tape was digitized into a computer filtered and enhanced
36:01now investigators had the clue they'd been looking for
36:12it was clear to us that there were they were really experiencing a problem but the
36:19airspeed and altitude and the airspeed and altitude indications in the aircraft are strictly a function of what we call the pitostatic system
36:34the pitostatic system is found on all aircraft large or small
36:49external ports measure outside air pressure to provide data on altitude and speed
36:55if these ports are blocked the plane's computers receive false data and generate false warnings
37:02but why these ports will be blocked was a mystery
37:06robotic vehicles searched for the missing piece of the puzzle
37:10what they found stunned investigators
37:14captain Schreiber static port was completely blocked with the tape
37:21investigators now learned what happened
37:26just before aero crew 603 lifted off from Lima
37:29maintenance workers clean the aircraft
37:32a worker covered the static ports with tape to protect them
37:35this is standard procedure
37:38but he forgot to remove the tape
37:41it was a small oversight
37:43with catastrophic results
37:48the taping was never removed
37:50and when the airplane departed and started to fly
37:53nothing but trapped static
37:56sea level air pressure was sensed by those instruments
37:59and in matter of fact the airplane was climbing up the thinner air
38:02and the information presented on the instruments and to the air data computer was false
38:09which generated just totally non-normal readings
38:14the inspector who was supposed to quality check his work did not do it
38:19and the supervisor out on the line that night was not there he was sick
38:24and there was a regular mechanic who was filling that role
38:29he did not see it
38:31and the captain or the pilot in this case the captain did the preflight
38:35they do a walk around looking for just that kind of thing
38:38the captain did the preflight that night and he did not detect it either
38:43a little piece of paper with glue caused an accident
38:48but the paper and the glue are not to blame
38:53humans are to blame
38:55because humans use that tape in the wrong place for the wrong purpose
39:00another accident shockingly similar to aero peru 603
39:07had happened just eight months earlier to another 757
39:13in february 1996 189 people died when a german charter called bergen air
39:20crashed five miles after takeoff from puerto plato dominican republic
39:23the ntsb assisted in the investigation
39:30a survey of the wreckage revealed that one pitot tube
39:35the other critical part of the pitot static system was blocked
39:40as with aero peru 603 night was the pilot's worst enemy
39:48the bergen air pilot flipped the plane upside down
39:52before crashing into the sea
40:07the bergen accident clearly pointed out already that
40:10a crew will get confused by these cryptic advisories
40:16rudder ratio mark airspeed trim
40:19and not getting a clear indication
40:21what the underlying problem is
40:23which is a problem with the air data computer
40:25or anything which comes behind it
40:26the pitot tube or the static port
40:28these were effects which were well known to the manufacturer
40:31and so it happened again
40:33three months after the bergen air crash
40:37bulletins were issued to all airline carriers
40:40about the pitot static problems
40:42but aero peru had not yet implemented the changes
40:49the bulletins and the let's call it the fruits
40:53of the dominican republic investigation of bergen air
40:57had not yet reached aero peru at the time this accident occurred
41:02the peruvian government very correctly made a point of that
41:07in their report on the accident saying that
41:10they should have given more impetus to those recommendations
41:13to get them out to the industry quicker
41:16well it's very unfortunate
41:19but it comes back to the fact that
41:22communication in aviation
41:24is sometimes deficient
41:27there is not that high level of exchange of information
41:33between different operators
41:35as they are all competitors
41:36they do not exchange certain information
41:38and specifically not the one which is safety related
41:41and that's definitely the case in the
41:43in the aero peru accident
41:459700 but it is indicating too low terrain
41:50even if schreiber and fernandez had known about bergen air
41:53it may not have helped them survive
41:56given the high pressure of their situation
41:58it's easy to sit here in the 757 cockpit
42:02and play the monday morning quarterback
42:05having heard the bells and the overspeed warnings
42:09the ground proximity warning the stall warning
42:12it was very easy to do that and sit here
42:15and say what I would have done being an experienced pilot
42:19but to put yourself into the position of those two pilots at night
42:22they were in an extremely difficult situation
42:25to fly that airplane and recover from that experience
42:33two weeks after the crash
42:35Monus Albert joined dozens of grieving families
42:38seeking the remains of his brother-in-law and friend
42:41he finally identified them in a lima morgue
42:45I wanted to find them
42:49I really wanted to find them
42:51and one part of me
42:53didn't want to find them
42:55because there was this fantasy
42:57that if I don't find them
42:58maybe they're in an island with a
43:01with a drink
43:03and looking at some girls dancing
43:05I can close the chapter
43:09I can go and take him and have him buried
43:12and there will be a place for the family to go
43:15and put some flowers once in a while
43:19and say okay my brother-in-law is here

43:31the lawsuits began
43:41in November 1996 a Miami lawyer took on the case on behalf of 41 passengers and crew
43:47arguing that the manufacturer Boeing was liable for the accident
43:57Boeing has to foresee the misuse of their product
44:00in other words the manufacturer of a product
44:02is legally liable for the foreseeable misuse of their product
44:06if it can be corrected
44:08in other words Boeing builds the airplane with a
44:10with potential hazard in it
44:12that hazard is that
44:14in order to clean the airplane
44:16you have to cover the static port
44:17and if you don't take it off
44:19the airplane can crash
44:21I wanted them back
44:23and since I couldn't get them back
44:25at least I wanted the wives of the victims to get compensated
44:30how much is that worth?

46:25was the lowest cultured and the one that knew less about what could happen

46:44so that you don't lose the airplane because a maintenance man making two dollars an hour
46:48down in Peru makes a mistake
46:50it's foreseeable that that kind of a person is going to make a mistake
46:54that's human nature they're going to make a mistake
46:57and you build your system so you don't lose your fifty million dollar airplane
47:00because a maintenance guy makes a simple mistake
47:03Schreiber and Fernandez were also scrutinized
47:07veteran pilot Alan McLeod believes that in their situation
47:11he would not have attempted to land
47:13he would have continued to fly for as long as he could
47:16with the plane angled upward slightly
47:19and the speed set just above cruise
47:23experience has shown that if you don't fly the airplane
47:27when you're experiencing an abnormal situation
47:29and they certainly were
47:31you must fly the airplane
47:33just concentrate on flying the airplane
47:34and get the airplane under control
47:36first and foremost
47:37are we going down now?
47:39we have 370 knots
47:41if you don't do that
47:42the airplane is going to bite you
47:44and you're going to end up in more serious situations
47:47so I would fly the airplane
47:49make sure I was able to fly it safely
47:51if only by using the attitude direction indicator
47:54and normal power settings that I was familiar with
47:57and then eventually work my way back and get it on the ground
48:02pilots call it flying by numbers
48:04relying not on computers
48:06but on basic aviation and human instinct
48:09it always becomes a problem when
48:12pilots are reduced to be push-button lists
48:15the man-machine interface has to work
48:19and has to work as humans are to operate these airplanes
48:25they have to be designed appropriately for the use of a human being
48:30of course you can put in all digits and numbers and computer gimmicks
48:35and you can
48:37but still it's man who has to operate and survey it
48:40and I believe a human being should be in charge
48:43in 1999
48:45Boeing and Aero Peru decided to settle the lawsuits out of court
48:49families and loved ones received an exceptional settlement
48:53averaging a million dollars US per victim
48:56the damages were high because of the terrible way
48:59the passengers and crew on Aero Peru died
49:03we were able to show that a lot of the people were alive
49:06in a crash like this a lot of the people would survive the crash
49:09and then died of drowning
49:12there was no question in our minds that the people suffered terrible terrible terror
49:17and pain when this happened to them
49:20they were horrified they were awake they knew what happened
49:24the disaster helped sink Aero Peru
49:27combined with increased competition and rising debt
49:30the national airline went bankrupt in 1999
49:34Boeing increased training on pitostatic problems
49:38and issued new regulations about unapproved static port covers
49:42since 1996 there has not been another pitostatic failure like the one on Aero Peru 603
49:48the designers of these products the manufacturers of the products
49:55I know that they have to take safety into account
50:00they have to of course they do that because they know it's the right thing to do
50:04but they also know that if they don't do it there's gonna be somebody watching them
50:07that's going to investigate it
50:09that's going to find out why it happened
50:11and that they're going to be accountable for what they do wrong
50:14and that if they don't take into consideration safety
50:17they're going to have to pay for it
50:19well it the accidents made the aviation industry aware that something simple like a pitot tube
50:26or static port can cause a major incident or even an accident
50:32and even airline pilots were reminded that this is a vital basic thing
50:43and it may happen and this is the way how to prevent it to turn into a catastrophe
50:49the case was settled the industry repented and moved on
50:55such is the world of commercial aviation
50:58but it was little consolation for those whose lives were scarred forever
51:02by an insignificant piece of tape
51:04they suddenly say that guy doesn't exist anymore
51:08it's very hard to swallow that
51:11it's very hard to understand
51:14and it took me a long time to accept
51:21so the memory is still there
51:23and it will be there for a long time
