00:42nottingham hey outdoor i've chopped my fingers off one of the busiest a&e departments in europe
00:49carry us now now under more pressure than ever patients from last night are still here
00:54the last few years have definitely been a lot tougher so sorry about the weight it's okay we
01:01just feel like we're battling every day but we have to keep going we're the right
01:06place for the right people thank you a place where life you're right love i just wanted you
01:16and loss he's going to go to intensive care unfold every single day your mom is here
01:25we can't let the bad times destroy people whatever happens you've got to fight
01:31it's humanity that gets people through it's the love she's here i will i love
01:37i think we sometimes take the people we love for granted and we think they're going to be there
01:42forever but we need to hold on to those connections because you don't know when they're not going to be
01:48there anymore i love you probably don't say as much as i said
02:01the adrenaline rush of motorbike racing is second to nothing the start of a race
02:14is absolutely horrendous they can be doing 180 miles an hour and six inches 12 inches apart
02:22fighting for that same piece of time to make a poor decision at 180 mile
02:26an hour the consequences can be final
02:39i've witnessed some terrible scenes in racing over the years
02:47the sensation of watching somebody die is not something that i care to do
02:52again especially not my son
03:01hello queen's ed so he's come off his bike and then he's added
03:05it's a head injury how long have you been about 30 minutes a 28 year old man's being rushed to
03:12queen's medical center after crashing his motorbike at a british super sport competition
03:19he's being accompanied by a medic from the track young guy come off at darrington park he's been hit
03:24by another rider head injury with a deviated pupil what's going to perform rsi
03:30his dad was with him at the race the amounts of that morning was pretty normal
03:36it was a good practice session it started well he just overcooked a corner
03:41you expect broken arm broken leg broken collarbone but he was unconscious and not responding to medical treatment
03:52they were absolutely devastated it's the first time
03:58in my life that i've actually been so worried about losing
04:05major trauma fellow dr usman is on call to resus
04:08he's racing fell over whilst getting up got hit by another motorcyclist mainly on the head
04:16gcs dropped to three
04:19with motorcycles i think most of the time we are preparing for the worst
04:25because the body is just so greatly exposed
04:30it could be a head injury it could be a limb injury or it could be a multiple injuries
04:35you've got a job to do you need to be able to make those snap decisions and the right decisions
04:43at the right time so that you can save a life this is a trauma isn't it yeah
04:50so this is max max is 28. he had a low side incident so bike slid out under him as he was getting up
04:59another bike obviously had a glancing bloke he lost consciousness gcs three for about five minutes
05:05gcs is otherwise known as the glasgow coma scale so we use that to assess a patient's neurological status
05:12every normal person walking around has a gcs of 15 and so a gcs of three means that something has
05:19terribly gone wrong he's learned people look slightly larger than the right had a rightward
05:25gazing pie according to the paramedics on scene was he wearing a helmet when he was clicking
05:33hello max i'm just going to do some examination of you okay
05:38trachea central any pain there's no chest wall tenderness take some deep breaths for me
05:47good air entry bilaterally both eyes are four millimeters for the head injury case you can end
05:56up with bleeding you can end up with bruising of the brain it's going to cover you up for a second
06:01there might be certain areas or segments of the brain that got damaged which are responsible for
06:06memory so was it the actual race or was it the warm up memory loss can be quite disconcerting
06:15we need to know how much damage has been caused to the brain so that's why we need to get the patient
06:22into the scanner very quickly do you know where you are uh no i'll book this one now
06:32it's heart-wrenching when you're stood there and it's your boy
06:35you realize that some might have brain damage or all sorts of things go through your head
06:43and he's not only um my son he's my best friend
06:56ready ready max is having an urgent ct scan after a high-speed motorcycle crash at donnington park
07:13racetrack well it's specifically the head actually he got gcs3 when it happened and
07:22they were about to intubate him and he's got amnesia now as well doctors are concerned about
07:28potential injuries to the head and brain after he lost consciousness at the scene
07:34so we're going to start with your head and your neck stay nice and still there
07:43family's everything to me it's all everything everything revolves around my family breathe in
07:49and hold your breath being a father it's the biggest thing that ever happened to me in my life
07:56and it's the most important and the most precious i used to ride motorcycles when i was younger had a
08:02few falls nothing as traumatic as that though max was a very quiet child by the time he was two he was
08:11riding a push bike by time he was two and a half he was riding without stabilizers and it didn't matter
08:19how many times he crashed he used to pick his little push bike up and continue to ride so we knew then
08:27that max was a little bit a little bit special then i remember his first ride on a big bike as he called
08:34it and his joy and that was the start of max's racing career what we're going to do for the next
08:41scan is to ask you to bring both arms up above your head do you think you can do it
08:45by the time max was six he'd had his first race when he was seven he started his first championship
08:57i remember him getting his first trophy and watching his his face lights up
09:03okay so for this kind of mission he's going to ask you to breathe in and hold your breath okay
09:06at nine max was the british champion on the 50 cc gearbox championships and at 10 he was a 70 cc champion
09:18breathe in and hold your breath
09:23he was tipped to be a top 20 rider in the world breathe
09:30for me the love of motorcycles come from a long way back
09:33my dad loved motorbikes he couldn't afford to buy us one so he made one out of all the old scrap
09:41whatever he had kicking about in his workshop
09:44it was small enough for me to just about ride eight years of age and definitely the right size for
09:50my brother at 12. absolutely ready steady roll
09:53i used to race when i was younger but it was just not meant to be for me and when i had to stop
10:04it felt like i'd had something stolen off me really

11:25after slicing through his finger with a circular saw
11:27this time of year we get all this wood ready for winter because we've got a log burner
11:35and we never use a central heating hello gary all right how are you feeling it started to bang now
11:43like and you were wearing a glove yeah i was professional by wearing gloves
11:50well i'd got all the big bits done and it was just the little bits what were left not thinking
11:59i lifted it up too near the blade next minute my fingers were off
12:05so this is his finger do you have any ice other than peace
12:09someone to costa for ice yes please
12:11or not that's our best bet medical ice are you legit to anything gary yeah needles
12:16apart from me no no are you going to needle me no i'm going to have a look at your hands right

12:39the whole way around and there's just a sheet of calls and pay for his fingertips

12:51of you mate it's still oozing how much blood did you see coming out of your hand none it
13:01were in my glove and then i wrapped it in a towel and that were it i'm gonna pop some dressing on
13:07that's hopefully that's hopefully going to stop the bleeding a bit oh god
13:25just need to take some blood tests and we'll give you some morphine okay yeah
13:28i was born in doncaster they want a lot of out in them days but what you had you looked after
13:39i promise this felt me as painful as that me and my brother john were really really close
13:47sorry my dude oh you're all right you're all right there's only two and a half years between us
13:52all done a very good ass you're a good one if you see me brother you'd see me and if you see me
14:00you see my brother my peas are melting my finger

14:17uh the other one we don't know where it is might have dropped out then it might be might be around

14:32football and cricket in the street and once the police fetched us home because we've been on railway
14:38lines i mean the police never come to door again because they never caught us me and my brother
14:44we couldn't catch us specialist doctors have come to assess the extent of gary's injuries
14:56have your eyes tom one of the plastic surgery doctors yeah so the finger is in there do you x-ray
15:02can you take that and x-ray that as well okay they will do it if you yeah okay they'll find it very
15:08strange but they want that and that i'll have a chat with you about what our options are often with
15:14fingertips you can let them do their own thing and they're here quite nicely yeah and the other
15:18finger i suspect will be on the borderline for us having a go at putting it back on
15:25that is not as easy as it sounds no you don't understand it might not work
15:32you could go either way
15:44hello queens right right side of chest injury so you've got spinal pain chest uh right hip left femur
16:02right elbow right elbow the rest of robs yeah how long are you gonna be all right thank you
16:12amber yeah hi you could have an amber trauma call please to adults resource
16:19the 75 year old woman is being rushed to queen's medical center
16:23after falling backwards down the stairs at home
16:2535 female full flight of stairs yep spinal pain whole spine right-sided chest wall tenderness yeah with
16:35initial low sats where they were 89 on room air when someone falls down the stairs you would worry about
16:42head injury neck injury have they had any injuries to their chest has the patient sustained irreversible
16:50damage to the spine temp 36 2 and she has had one gram of paracetamol and one gram of tx air
16:59somebody who's elderly and more frail is likely to have more significant injuries than somebody
17:05who is a bit more robust with a trauma call time is really critical trauma is here
17:13the lady's husband was at home when the accident happened normally when i go to bed i leave a light on
17:24and i didn't do this particular night and that's why that's why she fell
17:31the scream it were horrendous i've never heard a scream like it
17:36it's something what will probably live with me till the till the night transfer and then hand over
17:49this is stephanie she's a 75 year old female she's fallen backwards top to bottom down approximately 14
17:56steps she's complaining of c-spine pain pain all the way down her back right rib pain right hip pain and left
18:04femur pain okay and husband is husband's just outside okay lovely lovely stephanie i'm just going
18:12to gently press on your chest all right tell me if it's painful at all is that a bit painful there
18:17so you've got some left anterior chest wall tenderness and where's your pain worse at the moment
18:23on my back on your back okay if a patient's had a trauma and presents with pain in their spine
18:30you have to worry that they have some bony injury to their spinal column all right sweet
18:35just nice head for us i'll just go all right darling and obviously your spinal column protects your
18:41spinal cord which helps with your movement and your sensation throughout the whole of your body
18:47just nice and calm for me darling should we get some morphine yeah yeah should we go with that yeah
18:52something that's happened that we can't reverse then their life has been changed forever
18:57alero can you request this lady a trauma scan c-spine is immobilized but complaining of full
19:03spinal pain was hypoxic on arrival sats of 90 on air stephanie i've just got some warm blankets
19:10she's shivering at you bless you do we just pop a little bit of oxygen on her yeah absolutely sweet
19:15i'm just going to pop some of these nasal specs on you and put you on some four liters all right
19:19yeah there we go sweetheart give me an hour lovely done so you're going through a ct scanner rock and roll yeah let's go
19:34it's hard to explain the feeling when you've been with somebody for 50 odd years
19:40i don't know what i do i just don't know how i would go
19:57at all
20:10so we're worried about chest is that right i had more than chest his right pupil completely swayed to
20:23the right and it was sluggish doctors are awaiting the results of ct scans after max came off his
20:30motorbike at high speed gcs dropped to three at one point we were going to do an intubation at the scene
20:37he lost consciousness when he was accidentally hit in the head by another rider he does have a
20:44little bit of amnesia because he couldn't remember what he was racing so whether there's a delayed
20:50later on i don't know that's something to keep an eye out for
20:56with a head injury it's very important to keep a close eye on those patients because they can deteriorate
21:02very quickly and there's a very limited amount of time that neurosurgeons have to fix a problem if
21:08there is one hello max just keep your head nice and still for us okay
21:12look at some tape don't worry max i won't wax your eyebrows
21:23i was just like max when i were in my early teens i wanted to be a world champion motorcycle racer
21:36you watch it on tv and you just think god i can ride quicker than them
21:39and he's a world champion so that must make me a world champion
21:45years later my brother bought a motorcycle and started racing so it was easy for me i just went with
21:50him we joined a club in leeds we went and raced at motocross right max are you feeling a bit tired
21:57yeah i just need to do a bit more examination of you okay so stay with me for a bit longer yes
22:04all right all right what we'll do is we'll slowly go around the different areas and then we'll see
22:08what's cooking okay yeah sure
22:12the first race i ever had everybody says oh you're you're gonna be throwing up and you're gonna be so
22:18nervous and i won it much to everybody's amazement and they all said wow you fluked it it won't
22:25happen and you race three times in the day and i won all three by the end of the day and then
22:30of course they're all saying oh you're cheating somehow any pain without that phone
22:39i was riding at armature level i wanted to go up to british championship level which i was getting
22:43ready to do and got sponsorship i started doing a lot of training i was getting there but then when
22:52i was racing i kept passing out and they couldn't understand why and you can feel this yeah and this
23:01side yeah is there any difference between the two no do it twice and then they stopped me racing for a
23:06little while while they investigated any other parts wild war just want to stick your hand out for
23:13me yeah i went to the hospital and i was diagnosed with crohn's disease
23:23the consultant said to me we're going to remove part of your bowel and reattach your small intestine to
23:29it and it would be three years before i could train again realistically so the dream to be a world
23:37championship contender ended okay let's bend your knees please let me see it's very hard but the next
23:49best thing is is watch your son maybe make it all right we'll get an extra of your right elbow yep and
23:59also your right lower leg hopefully they won't show any fractures but you've got to make sure pain's
24:04not bone related yeah the nature of high speed impact means that it's likely there's multiple
24:13different forms of injuries affecting different parts of the body one would hope that over time
24:19they will be able to get full functionality back but it varies from person to person and it can cause
24:25life-changing issues for the patient
24:45breathe in and out through the straw it's going to make funny noises and we're going to take big deep
24:49breaths out good breathing all the way down to our belly okay there we go nice slow big deep breaths
25:03kian you all right what's this you're a bit spaced now mate am i real
25:11that's the gas and air that we use to get you feeling all spacey okay right you relax back for me
25:15so we're just going to move the board that you're on so you won't move two boats
25:41okay stephanie is having an urgent ct scan to assess the damage to her spine after falling backwards
25:49down a flight of stairs ready steady slide
25:58her husband james is in the relative's room so at the moment she's in the ct scanner okay so she's
26:05having a scan from her head to her pelvis that will tell us if there's been any injuries internally
26:10okay and then as soon as she's back from scan i will get you through to see her
26:17we'll do your scan of your head and your neck first okay yeah
26:24when i was younger i never thought about getting married and having a family
26:29and then i met stephanie oh
26:36she was bloody lovely aren't she
26:44first time i ever saw her we're on a bus coming back from nottingham and one of my mates says oh there's
26:51stephanie she's all right isn't she so i thought oh she's nice
27:01two or three weeks later i'm up the social club and who comes up
27:06so i went and started talking to her yeah yeah yeah walked around and she held my hand
27:13i couldn't believe my look
27:20are you all right now stephanie yeah so i'm gonna do the body scan now okay
27:25i think you can manage like that just for a few minutes
27:29we had a good date and another date and another day and i even used to dance then
27:34how are we doing stephanie are you okay okay yeah dancing apart until smooches quite
27:44not any much smooches she'll be all over me
27:54yeah well look we had a really nice court look we're a lovely courts here we were nice
27:59breathe in and hold your breath
28:01we got married on the 23rd of may 1970
28:09we decided to have children more or less straight away we had a girl and then a lad
28:15ian and jane well i would hover the moon breathe i got a job at lynn mccoyer everybody i knew worked
28:26at pit my dad and stephanie's dad we used to work there with the last blood i don't know
28:35we're almost done not much longer to go now
28:37we went there for security paid well we were able to sort of get a bit of money in the bank
28:48and then a certain lady appeared on the horizon and it ripped the heart out of her
29:02i've got some bad news kidder i've chopped my fingers off
29:20what you do chop my fingers off with chop saw well they're very nice
29:27no i can still wind the reels and i still can pick them pints up

29:42i'm just going to check your blood pressure quickly all right gary is awaiting an x-ray of his hand
29:49and amputated finger so doctors can see if it can be reattached what have they done with the um
29:55fingers they're in my cup they were still all in glove when ambulance come took them out
30:00and they took them out of the glove i'm just going to take you around for an x-ray all right
30:05i've got my fingers not so unbothered that all right so gary my name is gerald yeah i'm done with
30:16the cup i'll come back and uh have a couple of um x-rays yeah me and my brother we did everything
30:24together you know he didn't go off on his own doing something he'd always ask me and i'd do the same
30:30for him we always looked after each other
30:35all done there you go there my fingers look after him when he was 16 i was working in the
30:48shetlands on the oil refinery okay that's probably right i used to come home every six weeks and he
30:57used to meet me off at training doncaster and he used to carry my case for me and i used to fetch him
31:02some fags back this time he didn't meet me just get your phone out if you can that's it i went home
31:12and he was laid in front of uh open fire and me being me i kicked him in back and said what about
31:20my case and he just said he'd got these headaches all right all done someone's going to take you back
31:28the doctor to the doctors all right yeah we found the doctor and they sent him straight to doncaster
31:34royal infirmary and when it come to he'd got meningitis right here we go it sent him blind and mentally
31:43handicapped and now we're heartbroken with it
31:57so we're going to have a nice sunday evening before all this happened i went out for a drink and a bit
32:11of a dance oh a bit of a dance oh very nice i love dancing stephanie is awaiting the results of her ct
32:19scan after falling down the stairs doctors are concerned she may have fractured her spine
32:25do you work stephanie or retired retired now what did you used to be for work when i left school
32:31i was a machine is handy with a sewing machine yeah i've made all my kids clothes really oh wow
32:38couldn't afford them so made them gosh she's just in here

33:29so i went on strike to show a bit of solidarity with the lads i work with

33:44stephanie very worried about how we'd go for money she'd got a small job as a cleaner what paid
33:51a little bit of money and she made loads of closer kids she were brilliant
33:58stephanie i just need to check the temperature all right soon after i sold my car and things were
34:07starting to look a bit bleak that's when i went back it was a decision what we both made
34:12i didn't want to but we got no choice we had two kids and the mortgage rates at 12 and a half percent
34:23just couldn't manage
34:27right stephanie we've got some good news for you yeah so we've got the report now for the neck and the
34:38spine as well so there's no broken bones so we can take this off we can take the tape off and the big
34:45orange blocks sorry if it pulls your hair a little bit we can sit you up slowly have you got nail varnish
34:51on on my toes on your toes well it's nearly all gone now i might check it off last night
34:57and i thought oh no i'll do it tomorrow yeah oh dear all right let's get this out of the way
35:05when they've shot the pitch they want no work about a lot of people
35:10were on doll stopped on doll in hoottenham they were devastated
35:16oh my dog so i'm worried about you worried about me yeah tired what do you worry about you said
35:33stephanie always supported me always i'll be all right
35:39i'm the luckiest person on earth got a really solid relationship like we must have it's lasted 53 years
35:56stephanie will be kept in resus for further observation
36:09that you think that they're my fingers in there yeah in them
36:13what fingers are they them two these two oh mate what are you going to do
36:18you can't do it like that can i you still do that you said they'll be a bit shorter yeah that's it
36:24yeah you can have you can have one and a half or you can have a photo
36:30yeah you deserve the full two you can have half of one that's it yeah all right cheers
36:36gary is waiting for the results of his x-rays which will determine whether doctors can save his finger
36:47gary yeah right i've got some antibiotics got some morphine for the pain
36:51right i've also got technostab right you're going to put them all in there this one's
36:57muscle these two are in your iv line right when my brother got meningitis so he just started work he
37:06was happy so these are your antibiotics there's no needles involved in right okay he thought it
37:11was fantastic because you were earning a wage all right and then five weeks down the line that was it
37:18the impact was like him being born again my mom and dad fetched him up and like it were like going
37:32back to him being a baby so he had to be shown everything yeah feel all right yeah yeah yeah yeah
37:40it was hard i used to take him out down to pub and take him for a walk around block
37:46it'd have been a fantastic relationship if it was okay we'd have been done everything together he'd
37:53probably got into fishing with me but obviously them them days never come
37:59five years ago just one day he didn't feel right he went to hospital you know i spoke to him and
38:15everything they asked us to go out all right no three quarters of an hour later he passed away
38:26it was absolutely heartbreaking he never managed to do what he wanted to in life
38:32pain all right yeah it's going out pain good it's going well i'll keep popping me i didn't make sure

38:48it's left in this finger it's gone on there oblique laceration in the end which is a pretty
38:52straightforward fingertip injury would be fine to deal with plastic surgeon dr tom has gary's x-ray
38:59results middle wise is amputated through the dipj with a reasonable sort of there's a
39:05voter laceration for most for a little bit towards the pipj yeah yeah i think so
39:12you know we sort of said earlier we wouldn't really know until the x-rays whether it was even
39:24going to be a go in terms of putting it back on in the first place yeah yeah um it's probably a bit too
39:30far down chances of success are relatively slim and the amount of rehab you'd have to do
39:38to probably not get a great deal of function back right if it were if it survived yeah it's probably
39:43not worth putting you through yeah so our plan is to use this stuff on the back to pull it over pull
39:48it over it might be a bit so we can't get the skin across in which case we'd have to think about
39:54taking a bit of skin from somewhere else like a little skin graft or something yeah yeah um but
39:58we don't know until we can get in and have a look so we could get it done for you tonight
40:03yeah if it all works out logistically because we'll do it over at city hospital right what i
40:08want to go and ask you would i be able to go on holiday on saturday where you're going mexico the
40:14issue with going on holiday is that have you if you had a skin graft we'd have to check in a week
40:19and you wouldn't be here we'll try and work it out yeah when my brother got meningitis it taught me
40:28that you've got to live live your life to the full work hard play hard and go on holiday and
40:36everything we'll get you sorted out okay then thank you so i always live for today
40:44if tomorrow comes it's a bonus that's why i look at it now
40:58so uh we've got your ct scan results back it's all clear so you can get these off and sit you up
41:23okay yeah sounds good all right just keep your head still floras
41:33we can set you up now okay awesome just gonna do that slowly mate because you can feel a bit sicky
41:38when you've been lying for that for so long okay the a and e staff they'll keep you here for
41:44observations and then once the x-rays come back if there's anything that pops out they'll give us a call

42:00max's dad dave and his uncle michael have arrived at the hospital

42:06hey up there in chest how are you feeling sunshine rough rough everybody in the paddocks sending
42:15years past you were out for 10 minutes everybody's brought down in tears you were devastated they
42:26thought you'd copped it properly well i'm still here
42:34i wonder what happened don't matter what happened as long as you're on that
42:38good why did tom finish well it would practice weren't it we didn't race oh yeah we didn't make the race
42:45hello guys my name is umar so max he's had a ct scan yeah from top to bottom which is all clear
42:57yeah he's just we're now waiting for a few x-rays okay apart from that concussion is difficult to
43:04tell in the first 24 hours yeah symptoms develop after 24 hours roughly you won't feel very nauseous
43:10yeah but you should not be sick you will feel very vacant you will not be able to focus on things
43:18but if you can't see at all that's not acceptable yeah now concussion it's not
43:25life-threatening however if you bump your head again within two weeks
43:30on the same spot that can be life-threatening
43:34you need to be very careful you don't get another head injury
43:38would you remember all of that yeah because i don't think you'll remember everything i have
43:41remembered thank you thank you cheers
43:48it's very difficult as a parent when you know your son or your daughter's going into a sport
43:52whether the consequences can be final
43:56max knows every time he gets on the bikes the risk same as any other rider
44:00death is the ultimate price to pay for what you love
