Transcript
00:00I'm tired of being fat. I can't control my weight anymore. I can barely walk. I'm bad red. I don't
00:10want to continue my life like this. I can't believe I let myself get this way. I can't even lift my
00:16legs up. I'm really scared. Doctor Now, you are that change that I need. I don't want to lose my
00:23life. Doctor Now, I need your help. Nothing has worked for me. Please help me. If you want to live,
00:30you need weight loss surgery, but you're going to have to do your part. I won't be stopping now.
00:35That'd be your next success story. Ready to go. Ready to start a new healthy journey. If we're
00:40on this other, we're going to be on it. So we're going to do this. I am ready to take on this weight
00:45loss journey. I'll make the changes for my family, for myself. We want to be able to go out and be
00:50free. For once, I feel like I'm worth it.
00:57Nephi, a lot about your life needs to change. Get ready to work hard if you want to change your
01:04life. This is a big step for me to lose the weight. Now, let's kill us, too. Surgery or food?
01:09Food abuse is as bad as poison.
01:17Nephi. Morning, Papa.
01:20Hey. You need to go. Hey. Hey. Hey.
01:27Get up.
01:29Being 600 pounds is a real struggle. I'm always dealing with back pain, just overall joint pain.
01:38Even breathing feels heavy some days.
01:41I may be 25, but my body feels like it's 85.
01:46Just getting dressed is a battle for me.
01:48Part of me wants to change, but part of me is scared.
01:54What if I fail?
01:56Pretty much my whole world is inside this room.
02:00I just sit here, play video games all day.
02:04Eat, sleep, play video games.
02:06There's so many stuff I want to do with my life.
02:11Change sounds good when you're sitting on the couch, but it's hard when you actually have
02:16to get up and do it.
02:19I know if I don't do this now, I probably won't make it to C30.
02:23What are you doing?
02:26What are you doing?
02:26What are you doing?
02:27What are you doing?
02:27Nothing.
02:28Nothing.
02:29Just got off.
02:30How was work?
02:31It was good.
02:33Well, I'm hungry because I didn't get to take my break today, but it was okay.
02:38Nephi's really funny.
02:40I met him in 2021.
02:42Nephi actually messaged me on Instagram.
02:45And at first, I didn't answer because I didn't know who he was.
02:50What are you doing?
02:51I'm going to get you with my friends.
02:53We just started talking from there, and he was like, so what are you doing?
02:58Or what do you do for work?
03:00You know, he was just trying to get to know me.
03:02Then I asked him if he wanted to hang out.
03:05Scoot over.
03:07Scoot over.
03:08It's a single-seater.
03:09Oh, we're going to turn it into two.
03:11My boy.
03:13Every other day, I was over here.
03:15Once I started coming into the house, once I started sleeping over, he started feeling
03:20more comfortable with me.
03:21I would ask him little things, still trying to respect, you know, the boundary of him
03:26not opening up himself, because I worry about him a lot.
03:32Nephi, what are you doing?
03:34No.
03:34He's playing.
03:36Nephi, get out of the play.
03:39And I'm...
03:40Please, bring me your lovely clothes.
03:43Go.
03:43Floor is very, very good for Nephi, because Nephi is shy.
03:48I don't like it.
03:50He stays home most of the time.
03:52Most of the people that Nephi talks to are online.
03:57He doesn't like going out.
03:59But Floor asks, and Floor insists he's going out.
04:03Nephi will go.
04:04So the things that my mother does for me is, she does the cooking, she does my laundry,
04:13she puts my socks on for me.
04:16Your toes, nails toes, and everything.
04:20She does a lot for me.
04:25Anything else?
04:26With the dogs?
04:27Yeah.
04:27A couple of things my mother has told me about, me about my weight.
04:32You know, she tells me, I need you in my life.
04:36I need you to be here until you're 90, 100 years old.
04:42She doesn't want me to die at a young age, and neither do I.
04:46Now go back to playing GTA.
04:48I love being in front of the TV playing video games.
04:51All right, guys, I'm back.
04:56Nephi?
04:57Yeah.
04:58Do you want an orange?
04:59No.
05:00Want a banana?
05:01No, let's go get some real food.
05:04Well, what do you want?
05:05Some fast food.
05:07Fast food?
05:08Yeah.
05:09You said real food, my boy.
05:12That's one thing about Floor.
05:13Whenever I'm craving something, they're like, nah, you can't eat that, bro.
05:17I'm hard-headed.
05:18I don't like dieting.
05:20But I'm willing to make a change.
05:23It's hard sticking to a diet.
05:25I was, I don't want to stick to it for like a week, week and a half, and just go back to eating the same way I always ate.
05:39When I was born, I was chunky.
05:43My mom says I was a 10-pound baby.
05:46After one more, he doubled the weight.
05:50And he continued and continued, more big, more big.
05:53Every doctor tell him, let's wait, let's wait, let's wait.
05:58Please, let's wait.
06:01When I was 10, my father got deported.
06:05All I remember is that he took me in bed.
06:08He said, I'll see you tomorrow.
06:09The next day, I woke up.
06:11I called to my mother, what happened, why she broke to me the bad news that my father had been deported.
06:19I would go to sleep and just dream about him sometimes, that he was back at the house.
06:26I would wake up, expecting to see him, but he was nowhere to be found.
06:31Just growing up without a dad, it does affect a child.
06:34Maybe because I was just too young to understand what deportation was.
06:39Because I didn't cry, I didn't go through no certain emotions.
06:46After my father had been deported, I kept gaining weight.
06:50It was almost impossible for me to just stop.
06:53He tried coming back, and he was caught.
06:57Border Patrol told him, you can't come near the border for the next 10 years, and now you're going to be in jail.
07:03So, he just gave up.
07:07Growing up, I did realize I didn't need my father in my life, because I would go to parks.
07:13Then I see father and son moments.
07:18To this day, I do get a little emotional.
07:23So, I was around 13 when my people from Mexico called me and told me that my dad had passed away.
07:31They think he died of a heart attack.
07:36I never got to visit my dad, because I didn't want him to know the things I was going through.
07:42Every time he tried to video chat me, I'd tell him I was doing something.
07:46Or I wasn't in class.
07:48So, I would just text him, pull up a random excuse.
07:52I didn't want him to see me like that.
07:54I knew it was a mistake not to speak to my father on video when I had the chance.
08:01I can't change that now.
08:05When I was 14, I was around 300.
08:09I would get bullied for being overweight.
08:11So, I would constantly get into fights.
08:14Led me to a one-week suspension.
08:17So, I was home, alone.
08:21I had a little bit of money in my pocket.
08:24I asked my godfather for a go-kart, so I could go to the corner store right here in my neighborhood.
08:28But on the way back from the store, there was like a really big hole.
08:33I didn't see it.
08:34I hid it.
08:36I flipped in it.
08:37I walked out of there limping.
08:39Not sure where it hurted, but I just know from my lower back, it just hurted a lot.
08:46One day, he said, Mom, I can't get up the bed.
08:49And I said, okay, I'll hold you.
08:51Get up.
08:52And when he'd get up, he fell down in front of me.
08:56My mom was like, I'm calling no one.
08:59They told me it was the infection eating up my spinal discs.
09:02Staphylococcus damaged three discs.
09:06I'm going to cry like crazy.
09:10And if he holds my hand, he says, don't cry, Mama.
09:16Don't cry.
09:17You know what?
09:18God, God, he did this surgery for me.
09:22The doctors helped.
09:24Thank God.
09:26I stayed at an all-therapy hospital in downtown Houston.
09:30For three months, I was having nothing but physical therapy.
09:34After two years, he's paraplegic.
09:37I changed the diapers.
09:39I showered in the bed.
09:41It's so hard for me to see him in a wheelchair.
09:46It took a while to recover from the surgery.
09:48I was in a wheelchair for a few years until my legs and lower back went back to being somewhat normal.
09:57I keep having problems because obesity is a big part of my life.
10:02Last year, while I was in class in barber school, I passed out.
10:06People were saying I was purple.
10:08When I woke up, the doctor told me that I had carbon monoxide in my lungs and I needed CPAP to help me sleep at night.
10:16To this day, I struggle with a lot of things.
10:19I'm experiencing chest pain sometimes.
10:23I'm afraid my heart has just stopped beating.
10:28My worst fear is leaving this earth at an early age.
10:32I'm only 25 years old.
10:34I don't want to leave this world without experiencing a normal life.
10:40Just live my life to the fullest.
10:42We're on the way to the doctor's now, his office.
10:48Um, I'm nervous.
10:50I'm excited.
10:52I'm a little bit scared.
10:53Because I'm afraid he might tell me that I'm too big.
10:58You know, that he can't help me.
11:01Nefi?
11:04How are you doing today?
11:05Good, thank you.
11:12626.
11:17Not what I was expecting.
11:19I don't, I don't, I didn't think I was going to weigh that much.
11:22I'm sorry for letting you down, Mom.
11:25I need to lose this weight.
11:27And turn my life around already.
11:30Hey, I'll do it.
11:31Good, nice to meet you.
11:33All right, you must be nefi, okay?
11:35Yes, sir.
11:36All right, so you are 626 pounds, and you're 25 years old.
11:45So how did you get to be to that weight, and what age you started having issue with your weight?
11:53Well, I've been overweight all my life.
11:55Since you were two years old?
11:57Since I was born, I was a 10-pound baby.
11:59When he was, like, 12 years, he come in with depression, and after then, he's paraplegic for two years.
12:07So when he was paraplegic, he can't walk, and you bring him food?
12:12I'm feeling sorry for him.
12:13He lost daddy.
12:14After then, he's paraplegic, so.
12:16So, nefi, what do you have done so far that has worked or hasn't worked for you?
12:23I can't stick to a diet.
12:25About a week and a half, I completely forget about the diet.
12:28I can put the diet to the side.
12:29So you go back to your old habits?
12:32Correct.
12:32Do you work?
12:34No, I'm unemployed.
12:36All right, so what do you do all day?
12:38Watch TV, play video games.
12:40And what do you do for food?
12:42Well, just wait till my mom cooks.
12:44Most of the time, eat at home?
12:46Yeah, but my brother may come back from work, and he has food.
12:51So everybody bring you food?
12:53Yeah, pretty much.
12:55Mom, do you go out and buy him some drugs?
12:59No, sir, never.
13:00Okay, but you go out and buy him some food, which is worse than a drug for him at this point.
13:06Mom, I never think that he does such drugs, but I look like a food.
13:12Well, food is a substance that is hurting him and is dangerous like anything else.
13:19If you eat the food to survive, that's good, but excessive amount is as bad as poison.
13:28Okay, sir, I understand now.
13:29Okay?
13:30I got it.
13:30So we need to take care of that, because if he gains a little bit more weight, he won't
13:35be able to walk.
13:36He's going to be bedridden.
13:38And then they end his life before even he get to be 30.
13:44You need to understand that.
13:45All right, so Nephi, do you have any goals in your life?
13:51Yeah, I do.
13:52What are you going to be?
13:53A barber.
13:54For that, you have to have a stamina to stand up and work around the people.
13:59At your weight, how are you going to get even closer to the person to cut their hair?
14:06Okay?
14:08Yeah, that's why I'm here.
14:09Okay.
14:10Mom, are you going to help him with what you're cooking for him and make sure that he is eating
14:17a smaller portion, a low-calorie diet that will be 1,200 calories?
14:22Yes.
14:23Okay.
14:24And also, I'm going to get you an nutritionist that is going to help you with that.
14:29Okay?
14:29Yeah, I love him.
14:30So, there is no bright horizon for you as long as you are in this situation.
14:38So, let's see if we can meet with the therapy and change that.
14:42Okay?
14:44But the first step is going to get him to lose 60 pounds for two months.
14:50And also, I want to give him a personal goal of getting a job, something that is going to
14:55help him not be eating food and playing game.
14:58Okay?
14:59Okay?
14:59Okay?
15:00So, if he does that, I'll lose 60 pounds in the next two months, and then we're going
15:05to consider him to be a candidate for weight loss surgery.
15:08Okay.
15:09Okay.
15:09So, I'll see you in two months.
15:11I'll see you in two months.
15:12I'll see you in two months.
15:13I'll see you in two months.
15:14I'll see you in two months later.
15:15I'll see you in two months later.
15:16I'll see you in two months later.
15:17I'll see you in two months later.
15:18Okay?
15:19I'll see you in two months later.
15:20I'll see you in three months later.
15:22ēnēēēēēēēēēēēēēēēēēēēēēēēē at age 200p장이 a 100pā i vēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēēē.
15:26I'll see you in three minutes later.
15:27If you need to see a test there.
15:29I'll fly anik row your 3 minutes of the Nopeist files, that's on the phone this I, on the
15:31here.
15:32Right now, half an one is here, is M'd here to the Only환.
15:34But that's not going to only take a lot of commitment and work from Nephi, but also his mom.
15:40She's enabling him, and Nephi seems to be mama's boy.
15:45Nephi is not going to be able to change unless the dynamic with his mom changes drastically.
15:52So it's very important that we work to break this dysfunctional dynamic between them before it's too late.
15:59When Nephi's home, he usually eat what I cook for him.
16:04When I'm busy and I have no time to cook something in the house, I order the food.
16:12There you go.
16:14Where's the orange chicken?
16:16No, you can take the orange chicken because it's chicken fried.
16:20You can't eat. No. No?
16:22Okay, let me get an egg roll.
16:24Flavor is fried, too. No.
16:27Check it, they have broccoli and beef.
16:29I don't like this.
16:30Eat it.
16:33You need some orange chicken.
16:34You want the orange chicken? Come and get it.
16:37I know I shouldn't be eating Chinese food like this, but it's so hard not to sometimes.
16:42I really just want what I like.
16:45So now you're happy.
16:47You need to be controlling what you eat.
16:50It's a big surgery. It's very, very important for you to save your life.
16:55So we need to do the surgery.
16:57Mom, quit getting on my butt. I'm working on it.
17:00So we have a nutritionist coming over.
17:02I'm feeling a little nervous because I always let food control me.
17:09So now it's up to me to control it.
17:12All right, is this good or bad?
17:14Bad.
17:15All right.
17:16Good.
17:17You're such a quick learner.
17:18Is this all we have or are there other things that you think may be bad in the house?
17:24In the house?
17:25Yes, in the house.
17:26Here?
17:27Yes.
17:28Here.
17:29This house.
17:30Is there anything in the entire house that we may also deem as bad?
17:35Um, just wait here. Just wait here. I'll go get it.
17:46I know now why you're locking the door.
17:50Yeah.
17:51You play Be Smart with this thing?
17:52How long have you been hiding things under your bed?
17:55It's been about three months now.
17:57I go to bed around 4 to 5 a.m. after playing video games.
18:01Okay.
18:02That kind of gets me into craving snacks at midnight.
18:06What is it more important?
18:07Two things.
18:08Surgery or food?
18:10What is more important for you?
18:11I chose the surgery.
18:12That's why I turned the chips in.
18:14I don't want to see you this again because seriously, it's this time.
18:18So I gave up my midnight stash.
18:21I feel like this time the change might work because I'm going to do what I need to to get that surgery.
18:28Five pounds.
18:30I really need to get on my exercise.
18:33I've looked at Dr. Now's book a couple of times, but I haven't really been doing much.
18:39And I do need to lose 60 pounds.
18:42I missed the therapy session that I was supposed to go to and that's because I overslept.
18:51I just didn't make it a priority.
18:53I mean, I think it's weird to just sit there and talk to a complete stranger about my problems.
19:01You are in 619.
19:13What?
19:14Maybe you work out and then you do it though.
19:18What are you doing wrong?
19:20I don't know.
19:22The skill is true?
19:24Yeah.
19:25No.
19:26No.
19:27No.
19:28I don't know what I'm going to do.
19:31I do not know what I'm going to do.
19:33And I had told him I was so confident in myself.
19:36I told him that I was willing to put the work in.
19:41Well, Nephi, for the past two months, you actually didn't lose much weight.
19:47Seven pounds is not much weight loss.
19:50I know I didn't reach the goal of 60 pounds, but I put the effort in.
19:56Well, it looked like a lot for you because you actually never tried to lose any weight.
20:02You know, if you don't want to end up dead in five years, you have to lose weight now because you don't have much time.
20:09Okay?
20:10Yeah.
20:11So you want to do this now and your mom wants to do this now too?
20:16It's not up to my mom.
20:18It's up to me to get serious with myself, lose the weight so I can get approved for the surgery.
20:25Okay.
20:26So what do you think would happen in the past two months?
20:31Well, doctor, sometimes I give him food, extra food or something because I'm feeling sorry for him.
20:39And I want to see him be happy.
20:42So making him happy and giving him the food is helping him to die quicker.
20:49Is that making any sense?
20:52No.
20:53I need to learn how I carry him, how I cook it, how I serve it, how I do it.
20:59So I want to both change and make sure that both work in the same line and make sure that he understand that food is something that he need to restrict himself to certain amount and not overeat because next time it's going to be his last chance.
21:17So you didn't go to therapy, did you?
21:20No, I didn't.
21:21I actually missed the appointment.
21:23So, Duffy, you're sitting at home doing nothing.
21:28Did you find a job?
21:30I don't know.
21:31I haven't gotten a job, but although I didn't get a job, I did go into barber school.
21:35Well, I'm in barber school for eight hours.
21:38I am away from the TV.
21:41I am away from the video games and also away from binge eating.
21:45All right.
21:46Make sure that you make to your therapy and make sure you get to your goal and lose 60 pounds for the next two months.
21:55All right.
21:56I will reschedule the appointment.
21:58All right.
21:59So hopefully we see in two months 60 pounds lighter and make sure that you stick with your therapy goal and look for the job.
22:10I know you're going to school, but looking for the job also to help in the household.
22:15Okay.
22:16Thank you, sir.
22:18They both need to get motivated to take this more seriously.
22:22Or Nephi's health isn't going to improve and he's going to stay on the same dangerous path.
22:28Hopefully therapy with both of them will start to change their dynamic and destructive behavior.
22:34But we're not going to waste time and resources on Nephi if he wants to play games and just give me lip service.
22:41So he needs to have a good amount of progress and meet his goals by the next appointment.
22:46Or that will be it and he will be done with my program.
22:52Okay.
22:53So I came to visit Nephi today and turns out he was actually scheduled to work.
22:58So I got this part-time job.
23:01So how about the bills?
23:03I'm delivering packages.
23:05When I deliver the packages, well, you know, I get a little exercise.
23:09I get to get out of the truck and just walk to the doorstep.
23:13All right.
23:14So I got my meat, the cheese, and I got the broccoli.
23:21It should be quick and healthy and around 400 calories like Dr. Nat wants.
23:26You know, it's going to be hard to eat healthy on the bright side.
23:31This kind of food doesn't make me want to overeat.
23:35So I'm about to have therapy.
23:38I don't know what to expect.
23:39I've never had this kind of therapy before.
23:42I guess just trying to figure out why I do what I do when it comes to eating an excessive amount.
23:51Hi.
23:52I'm Joel.
23:53Thanks to meet you.
23:54Tell me about your background.
23:56Well, you know, I grew up with just my mom, my dad, and my brothers.
24:02When he got deported to being 15 years old, I felt like I got even bigger.
24:09I appreciate y'all sharing that with me.
24:12I like to speak with your mom, like, one-on-one for a few minutes.
24:16And then I'll bring you back in and talk to you alone if that's okay.
24:19Yeah, that's fine.
24:20Say he gets the bariatric surgery, and he still doesn't like the salad.
24:26What is the next step?
24:28Because that surgery can be so good, but it can be so bad, too, if you don't follow the things that you're supposed to follow.
24:35How are we going to get it so, you know, he's not hurting himself?
24:39Because I know you feel like you're helping him, but I don't think it's helping him.
24:44I think it's hurting him, and we need to help him grow.
24:49How do you feel about that?
24:50I need to change first.
24:53Small things, small things, heal myself, you know?
24:57I think that your therapy goal from this point forward is I want you to stop making those big meals for him
25:05and let him be kind of more independent.
25:08It's about giving him space to grow into his own life
25:11and giving yourself permission to let go and relax just a little bit.
25:16Mm-hmm.
25:17How do you feel about that?
25:19I need to do it, you know?
25:22You need to release some of that stress.
25:25His stress is your stress, and that's not good.
25:27I know we talked about, you know, your dad, you and your dad's relationship and his deportation,
25:33and unfortunately, his death.
25:36Can you walk me, like, through that again?
25:39The entire time, we weren't really in contact.
25:42I mean, why you didn't answer?
25:44Because you felt like your appearance?
25:47Is that why you didn't answer?
25:48Yeah.
25:49I felt like I would let him down, just him seeing me, the amount of weight I put on.
26:00Okay.
26:01So it's not like you didn't want it, but you just didn't want to disappoint him, right?
26:06Mm-hmm.
26:07Okay.
26:08How does that make you feel now?
26:10Like, you could have had that relationship with him, or he was trying.
26:14I know I was messed up on my part by not just answering none of the calls.
26:20I know I messed up.
26:22You were a kid, and then you didn't want to disappoint him because, you know, he probably held you to a high standard.
26:27You didn't want him to see you like that.
26:29That's a valid response.
26:31So after he passed away, what was your relationship with food?
26:35Did it get worse at that point?
26:37Food was my escape from what was really going on with me.
26:44Some people, when they grieve, they go shopping.
26:47Like me, when I grieve, I shop.
26:49And then some people, you know, they eat.
26:52And I think that's your coping mechanism.
26:55Yeah.
26:56And I think that we have to identify healthier coping mechanisms.
27:02Because if not, it's just, it's going to end bad.
27:06Like, the pain you stuff down has been eating at you for years.
27:10But if you don't talk about it, it'll never heal.
27:14That's true.
27:15You have to give your mom a break.
27:17So, here's where we start.
27:20I want you to take over all aspects of your food.
27:24Like what you eat, how you get it, how you prepare it.
27:29So, if you mess up, it's not on her.
27:32It's on you.
27:33Yeah.
27:34There's something I want you to do.
27:37I want you to record a video message to your father.
27:41I want you to tell him everything, you know, you never got to say.
27:46Okay.
27:47So, I'm working on my therapy goal.
27:49And, uh, dedicating a video to my father.
27:53I'm saying things I wish I would have said when he was alive.
27:56Now I'm brainstorming, working more, more things I've, I can add to the list.
28:03And then finally make the video.
28:06Hey, Dad.
28:08I don't even know where to start, man.
28:11I haven't really seen you since I was 10 years old.
28:15I didn't get to say goodbye.
28:19And I've carried that every day since.
28:22I didn't want you to see me like that on our video calls.
28:27So, I just skipped them through.
28:29Maybe you'd be embarrassed.
28:31So, I disappeared.
28:34And then you were gone.
28:37I never got to fix it.
28:40I never got to show you I cared.
28:45That I was still your son.
28:51I've made a lot of bad choices since then.
28:56I let life happen to me.
28:59Instead of fighting back, I gave up.
29:04But I'm trying now.
29:07I want you to know I'm sorry.
29:10And I miss you.
29:12And I wish I had told you that sooner.
29:15I think.
29:17No.
29:18I know.
29:20You'd still love me.
29:23I think you'd want me to keep going.
29:26So, I'm going.
29:28I'm going to get healthy.
29:30I'm going to pass those barber boards.
29:33I'm going to become someone you'd recognize again.
29:37I love you, Dad.
29:39I'll send you another update when I've turned the corner.
29:44Third visit at Dr. Now's office.
29:48Are you excited?
29:49I'm excited.
29:50Do you think you lost the weight?
29:52I think I lost the weight.
29:54I think I lost 60 pounds, if not somewhere close to it.
29:57He's been really strict on his diet.
30:00We're going to see what that scale says.
30:02And hopefully I get approved of surgery.
30:09That's 553.
30:26You lost 66 pounds.
30:30Yay!
30:32Oh, yay!
30:34We did it, puppy!
30:35Thanks.
30:37Yeah.
30:42So, you're 553.
30:44You lost some weight a whole lot more than the first time, huh?
30:48Yes, he did, sir.
30:50All right.
30:51What made difference this time?
30:53I went to therapy this time.
30:55I did exactly what Jewel told me.
30:57So, did he give you any call with the therapy?
31:00To make a video to my dad.
31:03And, uh, stuff like that.
31:06And, uh, my mom has also been doing well.
31:08You know, helping me cook.
31:10Okay.
31:11I followed the instructions on your book.
31:13And I was just disciplined, took it more serious,
31:16and started shedding the weight.
31:19That's good.
31:20That's good.
31:21So, did you get a job?
31:23Yes, I got a job now.
31:25I'm making my own money.
31:27I'm not spending it on food.
31:28I cook for myself now.
31:30Well, that's great.
31:32Enfia, I met your mom.
31:33And who is this other person with you?
31:36Oh, this is my friend Flor.
31:38We've been friends for more than five years.
31:40And, uh, is she helping you with your weight loss?
31:43Yes.
31:44I try to get him to be a little more active, just walking around.
31:48I think it's the baby steps that count for the big change that he's able to make.
31:53All right.
31:54So, that is very positive for all of you.
31:57I'm very proud of your progress.
31:59So, since you start a good progress and you lost 66 pounds, I'm going to go ahead and approve you for weight loss surgery.
32:07Thank you so much, doc.
32:09Okay.
32:10Under one condition that you will continue to lose the same amount of weight.
32:15So, I'm going to set up your surgery for one month from now.
32:19And you need to lose 30 pounds in one month.
32:23And if you come back and you have lost 30 pounds, we're not going to go ahead with your surgery.
32:30You think you can do that?
32:31Yes.
32:32Okay.
32:33All right.
32:34If you need anything, give me a call.
32:36Yes, sir.
32:37Thank you so much.
32:38All right.
32:39I'll see you all later.
32:41I am presently surprised to see such a turnaround with Nephi and his mom after such poor progress the first time.
32:49But Nephi still needs to show me that this is not just one-time thing.
32:54So, I still need to see some good progress.
32:57As long as they do that, we will move ahead with surgery next month.
33:07I'm nervous.
33:08I'm a little scared.
33:10When Nephi was 12, I sat at a hospital waiting for Nephi to get out of surgery.
33:17He's a lot older and he's a lot heavier now.
33:21Maybe twice he's heavy.
33:23I worry for him because I know the surgery can be dangerous for some people's his size.
33:31He got me blood for surgery.
33:32He got me blood for surgery.
33:33So, if probably something may go wrong.
33:34I have faith in God.
33:35I'm hoping that everything will turn out good.
33:38Faith in God, and hoping that everything will turn out good.
33:515.19.
33:53Well, that's good.
33:56Yep.
33:56And so it looked like you lost 34 pounds from the last time.
34:02Yes, sir.
34:02He's shown me that he's ready for this step today.
34:06He lost just a little less than a half of what he lost in last appointment.
34:11But there's still a good amount of weight loss, and slowdown is to be expected.
34:17I'm ready.
34:20Overall, he is over 100 pounds down since he started.
34:25So I'm confident he's ready to do this.
34:28We're going to perform a gastric sleeve on nephew today.
34:31For this operation, we are going to remove about 70% to 80% of his stomach.
34:37And what this surgery does, it will give him opportunity to eat less and be able to lose more weight and maintain a healthy weight loss.
34:46All right, let's bring the robot.
34:58Nephi is in surgery already.
35:01We are waiting to hear back.
35:03We're hoping everything is okay.
35:05Me, personally, I'm a little scared.
35:07But, you know, we're keeping our faith in God and hoping that everything turns out good.
35:12And for me, one second is like a one hour.
35:18Okay, hold on just a second.
35:19Hold on.
35:21You get a clamp and hold it.
35:23Nephi's surgery went very well.
35:27We were able to remove 80% to 85% of his stomach and do his neviotrectomy.
35:33And this will give him an opportunity to be able to control his eating habit much easier.
35:39And turn the room right on.
35:42So this is going to be a good tool for him to stay on track, to keep going and lose a lot more weight.
35:48As long as he stays focused and do what he needs to do.
35:53Weight loss surgery is not a magic fix.
35:56It still takes a lot of hard work.
35:58And a big part of that is dealing with emotional issues that are behind the pathological eating.
36:06And therapy is going to be important to help them do that.
36:10All right, arm down.
36:12All right, nice, slow, deep breaths, Nephi.
36:15Over the next one, Nephi is going to be on a liquid diet.
36:19And he should be able to use 35 to 45 pounds in that time.
36:25And after that, he should be able to lose 20 pounds a month until he gets closer to his target rate.
36:32They just still have a lot of work to do to make that happen.
36:38It's a lot that came off.
36:42Probably about half a pound.
36:43And this, that will be another pound.
36:45Like, complete a pound.
36:47I'll have to get used to it, but it feels good.
37:00So I've been really studying for my barber's test.
37:05I even took a practice test.
37:08And I did very well.
37:10I passed it.
37:11So today, I will be scheduling my barber exam for the state boards.
37:17And I'm feeling very confident.
37:19I think I might pass it.
37:20I do got to wait two weeks.
37:22But it gives me more time to calm my nerves and study a little bit more.
37:30Oh, we're here.
37:31About to go hiking.
37:32About to get some steps in.
37:33Going to be getting some sweat in, too.
37:36I'm going to be sweating.
37:38Hi, my name's Saul.
37:39I'm just a little younger brother.
37:41It's been fun going through the journey with him.
37:43Seeing him happy and being able to move a lot more is motivating for me and for him.
37:49Like, it just pushes us.
37:52Your shirt's already.
37:53It's loose.
37:53Imagine another size.
37:54It's five size.
37:55Five eggs.
37:56What's the goal?
37:57Three eggs?
37:58Two eggs, baby.
37:59Two eggs.
38:00Trying to get to two eggs.
38:01I was wondering, I was going to ask you, like, how is it not eating?
38:05I don't know how you feel like.
38:06Are you still hungry?
38:07I don't know.
38:07I guess I already got used to it.
38:09Just eating less.
38:11Got a good little sweating already?
38:13Yep.
38:14Healthy lifestyle is going good?
38:16Slowly, right?
38:17You haven't been, like, craving any snacks or anything?
38:21I do crave them.
38:23Like, what do you crave?
38:24Like, a bag of chips.
38:25No bag of chips.
38:26But I try to go to healthier options and obviously limit the intake.
38:32Okay.
38:32Are you more conscious of how you eat?
38:34Once I feel that instinct in my stomach that I'm getting full, I push the plate away.
38:39Like that?
38:40Like that.
38:41So how do you feel mentally?
38:42Do you feel like you would ever return to where you once were?
38:45Um, I feel like it would if I let it, but I don't plan on it.
38:50Okay.
38:50I don't plan on it, you know?
38:52I'm trying to be fit.
38:54I'm trying to be skinny.
38:54Your goal should be to be healthy.
38:56It should be to be skinny.
38:57It should be to be healthy.
38:58Sure.
38:58Have your pants shrunk a lot or your shirts?
39:00Both.
39:01Both.
39:01Like, which one more?
39:03Shirt.
39:03Shirt?
39:04I went from an 8X to a 6.
39:06Oh.
39:06How does that make you feel?
39:07How does that make you feel?
39:08It makes me feel happy, you know?
39:11It makes me feel like I made a very good decision.
39:15Okay.
39:16On getting that surgery.
39:17So I'm trying to keep pushing forward and not going backwards.
39:20I'm glad your mentality has changed because at the end of the day, that's all that matters.
39:23That's really the big step.
39:25Yep.
39:26Good job, dog.
39:26That's good.
39:28Tomorrow, 5 in the morning.
39:29No sun.
39:29Sun won't burn your neck.
39:30All right, so there it is.
39:34I'm about to hop on it.
39:38463.
39:40I'm cool with that.
39:41Nafi loves so much weight.
39:44It's amazing.
39:45He has more energy.
39:47He takes responsibility for his choices.
39:50And he's finally getting the life he wanted.
39:54I'm so happy for him and proud for him.
39:58And thank you.
39:59Thank you so much, Dr. Now.
40:01I know he's dead in Washington.
40:03And he's so proud of him.
40:06Hey, Dad.
40:07I just want to let you know I'm doing better.
40:12I'm going out more.
40:21Got a job opportunity coming up as a barber.
40:25Something I've been working hard for.
40:27I'm helping Mama around the house.
40:30You know, I help her out a lot.
40:31I cook for myself.
40:33Try to get into cooking Chinese food because I don't remember how much you like it.
40:37I remember you picking us out.
40:39Chinese buffet every weekend.
40:40I'm losing the weight.
40:42I get on that scale.
40:44I see the numbers smaller and smaller.
40:47That's just my happy place right now.
40:49Mom wakes up in the morning and just sees me and says,
40:54You look thinner.
40:55Seeing that smile on my mom's face.
40:58You know, that's what I wanted.
40:59I wanted my mama to be proud of me.
41:01I just wish you were here to see me.
41:04Turn my life around.
41:06I'm sorry.
41:07I never answered your video, Paul.
41:11I should have spoken to you when I had the chance.
41:14I should have told you about my problems.
41:16I should have told you what I was going through as a kid.
41:19Maybe you had an answer.
41:22Maybe you would have been a support.
41:25I know you will be proud of me more than anything.
41:30Just wherever you are, please watch over me.
41:35And just watch this progress.
41:38But I wish you were physically here.
41:40So I could just hug you one last time.
41:43So I could just hug you one last time.
