01:13I'm reaching out to you because I've seen you help
01:15some of the worst kids I've ever seen.
01:17I've been struggling with my weight, and I really do not
01:20want to die at only 20 years old, so I need your help, Doctor Now.
01:24Every attempt that I've taken to lose weight has always failed.
01:27I live with my mom and my sister, and all I do most days
01:30is sit and eat fast food, or I play games.
01:34I never thought I'd be this big.
01:38I've always been, like, tall, but, like, I've always been fat, too.
01:43But now, my weight is, like, an all-time high for me,
01:46and that keeps me from having a normal life.
01:50Butter makes everything so much better.
01:53We help Caleb in a lot of ways because, you know,
01:56he can't do a lot of stuff.
01:58Sometimes I help Caleb get his food, put his food in the air fryer.
02:02I get him water.
02:03We fill his cup.
02:05Sometimes get his laundry out, stuff like that.
02:07I don't think I'd still be living with my mom and sister at 20.
02:12I should be doing things on my own.
02:14I wanted to go to college, but I can't fit in the desk.
02:17So I'm trying online courses.
02:19I don't even have my license.
02:20But even if I did, I still wouldn't be able to go out.
02:23I hate seeing him like this.
02:26It just breaks my heart because I think him not being able to do things
02:30that he used to do affects him a lot,
02:33to the point where it becomes, like, painful sometimes.
02:37He is mobile for his size, but he doesn't really go out much,
02:41and he gets to stares and he gets to laugh,
02:44so then it's really hard for me to get him to go anywhere.
02:47I don't have any friends anymore except online.
02:50The majority of my friends, they live in Missouri or Texas.
02:54Play the game with them for a little bit.
02:56Being here in this house is pretty much my life,
02:59and I'm really ready to change that.
03:01But I've tried, so I need to get help before it gets worse.
03:09I think my relationship with food really started when I was four or five.
03:14I have three siblings.
03:16I have an older brother, an older sister, and a younger sister.
03:19But growing up, it was just me and my older brother, Christian.
03:22I was always hanging out with him, playing sports.
03:25Growing up, initially, Caleb and his siblings were very close.
03:28With my older son, he was really close to him
03:31because he looked up to him a lot.
03:33Early on, my parents, they weren't really getting along a lot.
03:36When I was four, they did break up.
03:39I still saw my dad from time to time whenever he had time.
03:42At that time, my mom, she was working a lot.
03:45She was working like two or three jobs to provide for us.
03:48So we started to rely on fast food, and I started gaining a lot of weight.
03:54Normally, whenever I work, I work overnight, so it's kind of hard.
03:57It's really easier to eat bad and cheaper than it is to eat healthier.
04:03The summer of fifth grade going into sixth grade, I was around like 308 pounds.
04:10As he began to gain even more weight, he would find food in the house,
04:14and he would just hoard it and hide it on the side of his bed.
04:17So then we sent him to a weight loss camp.
04:20I was there for the whole entire summer for two months.
04:25And when I got done, I had lost a total of 49 pounds.
04:29Losing those 50 pounds really gave me a boost of confidence that I needed.
04:33So sixth grade was a great time because it was the healthiest I had been at that time.
04:40And being able to play sports, it made me feel like great because, you know,
04:45when you join a team, it's like you're joining a brotherhood.
04:48But I still wasn't putting the right things in my body that I was supposed to,
04:51just still eating whatever I wanted.
04:53I was starting to gain again and put back on the pounds that I had lost.
04:57And that even got worse in eighth grade because of my legs.
05:00I was having an excruciating pain in my legs, and it was hurting, like, so badly.
05:04Like, it would hurt when I walked.
05:05And we went in to get a check, and they found out that I had Blount's disease.
05:10Blount's disease is a disease where your legs bow easily.
05:15And I had had it over my entire life, so I was getting bigger.
05:20And it got so bad that my knee and my leg were in a 90-degree angle all the way down to my ankle.
05:26So they decided that I should get the restructuring surgery on my leg so they can straighten my leg out.
05:33And that was the worst pain I ever felt in my life.
05:37Initially, Caleb wasn't this tall.
05:39When he had his first surgery, he was only, like, 5'9".
05:42So when they broke his leg and they straightened his leg, he went from 5'9 to 6'1", 6'2".
05:48And then when they broke the other leg and straightened it, he went to 6'5", and then he just continued to grow.
05:55During that summer of me going into my eighth grade year, I was immobilized the whole summer.
06:02I couldn't do much.
06:03So I just ate and slept, and that's when I started to approach the 500 mark.
06:09At that time, my brother Christian was in college, so he had already moved out.
06:15But my mom got a new job all the way in California and moved to California with my mom and my younger sister.
06:21And I started high school there.
06:23My diet at that point, I was eating whatever I wanted.
06:27I didn't know anybody out here in California.
06:30So when my junior year ended, I had gained up to, like, over 600 pounds.
06:34But then Christian had passed away in a car accident.
06:38And I remember it was like yesterday.
06:42When school ended, I had to go back to Missouri for summer school because I didn't pass geometry.
06:47So I was living with my dad.
06:50It was close to midnight going on June 13th.
06:52My dad, he had kept walking into my room, like, and he had just told me, he was like,
06:57Caleb, I'm sorry to tell you, your brother passed.
06:59I was just in shock, honestly.
07:01Like, this was the first time, like, losing somebody as close to me as my brother.
07:04The next morning, it was definitely a hard hit to me.
07:08After that, it was a spiral.
07:10I just fell into deep depression.
07:13You know, it was sad every day.
07:15Didn't really want to talk to anybody.
07:17The passion of my brother was definitely a main turning point in the weight gain.
07:22After he passed, I gained a total of 120 pounds over the next two, three years.
07:29The stuff in my face with every food I could get, not doing much at all.
07:33I went back to California and started my senior year.
07:36But then, six months later, my sister came in my room and told me that my dad had passed away.
07:41And he was asleep, he had a heart attack.
07:44I remember the night that my mom said she wanted to send us back to St. Louis for Christmas.
07:49She had told me to call my dad, let him know that I was going to be flying out there the next day.
07:53I shrugged it off.
07:54I'm like, no, I'll call him in the morning and let him know I'm coming.
07:56I really didn't talk to him a lot for two months.
08:00And just thinking six months ago, he was calling me crying, talking to me about his health and how I need to get together.
08:06And here we are, six months later, he dies from a heart attack in his sleep.
08:11It really makes you change your aspect of life.
08:14That moment, it was similar to how I found out about Christian past.
08:19I was shocked, like, I cried.
08:22I think the passing of both my son and his father was definitely an effect on him,
08:28because he looked up to both of them.
08:30It makes me feel really bad that I can't really do anything about it to help him,
08:34because he doesn't like to talk.
08:36He likes to just bottle everything up.
08:38You know, I talked to him about therapy.
08:41He was like, I don't know, Mom.
08:43You know, I don't really like to tell people about my feelings and how I feel.
08:47Around this time, I had gained up to 760 pounds.
08:52So, you know, I just thought, why not take a chance and get my health together for my dad and my brother
08:57and so I'm finally doing what is right.
09:00And I'm finally taking a chance on my health for the better or worse,
09:05because if I don't, then it is a fact that I couldn't like him.
09:10So I've decided to go see Dr. Now for his help before it's too late.
09:16All right, yeah, we made the doctor now.
09:19And I haven't weighed in yet, but if I think about where I weigh at,
09:24I think I'm around 750 or 760.
09:27If I'm anything higher than that, I'm going to be really upset.
09:31Okay, go ahead.
09:49Just got my weight. Feeling very disappointed in myself.
09:52Very angry about it, but it's the reasoning I'm here.
09:56So just ready to get the journey started.

10:11My mom.
10:12Good to meet you both.
10:14So, Caleb, you're 20 years old and you're over 800 pounds.

10:21Walk me through what you eat a typical day.
10:25If she buys, like, chicken nuggets or frozen, like, peach rolls or something,
10:30I eat, like, 20 of those.
10:31Or if she gets, like, frozen pizzas, I eat a whole frozen pizza to myself.
10:35I eat a lot of cereal, drink a lot of milk.
10:38And if I want to make, like, a breakfast sandwich,
10:41I make, like, two breakfast sandwiches with, like, four pieces of sausage on each one
10:44and, like, an egg and cheese or something.
10:47And then if she has, like, frozen wings that I can make myself, I'll season those up,
10:52make, like, eight of those with, like, some fries or something.
10:55Or, like, if she has, like, bought steaks, I'll make a steak with, like, a baked potato.
11:00Or, like, occasionally if I feel healthy, I'll make, like, a couple pieces of salmon and white jasmine rice on the side.
11:09So after eating pizza, burger, and fries, you think you just eat some salmon and you're healthy?
11:18Um...
11:19If you're eating 5,000 to 8,000 calories a day, there is no healthy meal.
11:24It's all extra calories.
11:25Yes, sir.

11:27So aside from eating, what do you do all day?
11:30So I usually, like, watch movies, play games, just stay in my room.
11:33So who supports you for that?
11:35Uh, my mom, if she does for me.
11:38So they're your enabler to get you to eat and do nothing and live in the house?
11:45Uh, yes, sir.
11:47What do you do?

11:49I'm a travel nurse, but I travel specifically, specifically in California.
11:53So a lot of the times I work around home, but a lot of the times we do eat out.
11:58I'm going to be honest, because sometimes it's easier to, like, get something fast.
12:03Because, like, he doesn't drive, and that's been an issue, too.
12:07The issue is, is that we have to find a car big enough for him to fit in comfortably to take the driving exam.
12:14So it's trying to get him to be, like, more independent, because I ain't able to.
12:18I know I have.
12:19I'm not even going to lie and say that I haven't.
12:21So mom is busy and going to work.
12:25Uh-huh.
12:26And how do you get your food?
12:27Do you order it?
12:28Oh, I do the grocery shopping, of course.
12:30So initially, I'm like, what do you guys want?
12:33And so they send me a list or whatever it is, and then I get whatever.
12:37So it's groceries in the house.
12:39But then also, he has money that he gets paid every Friday, $200 a week.
12:43And in that $200 on Friday, by Monday, he may have $25.
12:47And I'm like, Caleb, like, what did you eat?
12:49Oh, I bought me a Kennedy food, so it's always me and Kennedy.
12:54You give him $200 a week?
12:56Yeah.
12:57It's 20 years old.
12:58He should be responsible for you.
13:00That's true.
13:01He should pay you $200 a week for being in the house.
13:06You know what?
13:07And I told him that.
13:08I told him his rent in California for his room, I could easily get $1,000.
13:11Mom, I got news for you.
13:13You're confusing love with giving him things.
13:17Yeah.
13:18That is not the real love.
13:20That you're just giving him more approval for putting more weight on himself.
13:25He should be 200 pounds, and he's 800 pounds.
13:29So he weighs four times as much as he should weigh.
13:34So you're helping him to kill himself with the food.
13:37I know, but it's just like I never, like initially I was like, am I cruel if I lock the food up?
13:44You know, like, how far was, you know, am I supposed to go to keep him safe?
13:52I just always told him, like, I need you to get it because I know that it's killing you.
13:58But it's like, I need him to understand that.
14:01It's like, I know it, and then I, it's not like, like, I've cut off where I don't do it.
14:07And then it's just like, I feel bad because then it's like, am I, you know?
14:11It's like, I don't, I know we need help because I can't do it.
14:15You shouldn't feel bad by cutting off his food supply.
14:19Well, not the food, but it's just for, well, the money is the food, so.
14:22That's where it goes.
14:24So this has got him to the point that he's comfortable with what he's doing right now.
14:30And this is not going to happen.
14:32His health is going to require tough love.
14:36We can get him to discipline himself.
14:39But if somebody give him temptation of giving him $200 to spend on anything he want to eat,
14:46what do you think is going to happen?
14:48Right.
14:49So how are we going to change that?
14:51Well, we just got to just make the sacrifices because it's life or death now.
14:56I mean, to be honest.
14:58Like, I never in my life thought that he would be 800 pounds and here he is.
15:03He is.
15:04And that is a very dangerous weight to be.
15:07You know, if anything happened to him, his body won't be able to handle it.
15:12Dr. Nell, even though I'm only 20, like, seeing everything that I could have done at a young age
15:19to just lose the small amount of weight and then now gaining more and more because I've been so weak-minded
15:26and just been so delusional about it.
15:29And then now seeing I'm 800 pounds at only 20, it's like, you have to change.
15:35Like, even if, like, they do their own thing, like, I still have to be on myself about it to stay focused
15:42and be able to do it and lose the weight I need to lose.
15:45Good.
15:46Well, let me ask you this question.
15:48If today you were 200 pounds instead of being 800, what could you do that you cannot do now?
15:56Play sports, be living a normal college life as a teenager, working.
16:02The driver?
16:03Yeah.
16:04I would be ahead, having my license and everything already, making my own money,
16:09not having to ask her to give me anything, and probably wouldn't be in the house at all.
16:16When you're 800 pounds, you have to eat, like, 8,000 calories to maintain that weight.
16:23If you drop it to 1,200 calories a day, you're going to be able to lose 100 pounds in one month.
16:30But the goal I'm going to give you is to lose 100 pounds in two months.
16:35Okay?
16:36Yes, sir.
16:37All right.
16:38So, you do any exercise now?
16:40Occasionally, we have, like, a personal gym downstairs in our complex,
16:44and I tend to, like, lift weights every now and then.
16:46How occasionally is that occasion?
16:48Like, once every two weeks.
16:51I'm not sure about that, but regardless, you're going to do some exercise twice a day.
16:57All right.
16:58So, I'm going to give you some homework.
17:00So, the goal is that you're going to eat 1,200 calories a day, which is going to be high protein, no carb.
17:07Okay?
17:08And we get you to see a nutritionist to understand the diet and what choices do you have to make.
17:14To get to 800 pounds at 20 years old, take a whole lot of pathological eating.
17:20So, with Caleb, it's going to be important that he start therapy immediately.
17:25He doesn't seem to have a lot of people to talk to since most of his social interactions seem to be with his family.
17:33So, having someone else with the therapy to help him talk about things will hopefully help him to start open up,
17:41to get the emotional issues that drive him to eat.
17:44Doing that is going to be important for his progress in the long run.
17:50Now, exercise is not going to be a big factor for your weight loss, but it's going to be a big factor for your stamina.

18:00So, I'll give you a goal of lose 100 pounds in two months, and I want you to set a personal goal for yourself.
18:07Who does clean the house and do laundry?
18:09My mom do it.
18:11So, what do you think about doing the chores and helping her on the house?
18:16Uh, yes, sir.
18:17It's not fair to her to come home and have to, you know, feed me and take care of me.
18:20So, I want to do more for her and make sure I'm helping her out the right way.

18:29And also, set you up to meet a therapist and they will give you a goal that I want you to work toward, too.
18:37And if you do that and lose 100 pounds in two months, then I think weight loss surgery will be an option for you.

18:55I got two months to lose up to 100 pounds before I can go back and visit the doctor now.
18:58So, that's nearly one or two pounds a day.

19:04As long as I be consistent and keep doing what I'm supposed to do, it won't be a struggle at all.

19:13I had my first therapy appointment with a therapist, and it was good.
19:17After your dad had passed away, how did that affect you emotionally and physically?
19:23My dad was really, like, my best friend, honestly.
19:25So, losing him, like, it really hit me, like, deep, and it was bad.
19:30And what about your brother? Were you and your brother close?
19:34Me and my brother, we were very close growing up.
19:36But he taught me how to do everything I know.

19:40And I know you mentioned, like, certain people that are there for you.
19:44Outside of your family, do you have any, like, friends that are there for you or anything like that?
19:49Uh, yeah, I don't have any friends out here in California with me.
19:52Since I've been in college, no, I haven't really made any new friends.
19:56You're in college online as well, right?
19:58Uh, yeah.
19:59Well, I know you have your family and stuff like that, but, you know, you gotta reach out and you gotta have, like, a community.
20:05Like, I know, virtually, it's easy to pick up on friends, you can find friends.
20:10But it's nothing like having, like, a friend to get you out and about.
20:14Mm-hmm.
20:15Because we wanna get you out more.
20:17I want you to be a little bit more social.
20:20This is real difficult to talk about my brother and my dad sometimes, so that was the hard part.
20:25My homework that she had me, that she wants me to do, is she wants me to get out and be more social and hang out with real people.
20:32But I also have some good news.
20:34I'm glad to say that I've already completed my personal goal with Dr. Now, and that was to help out my mom around the house.
20:40As you guys know, my mom's a nurse.
20:42She works 12 hours, three, four days a week.
20:45Recently, my mom, she took the day off, and she had to watch my niece and nephew.
20:51I thought, hey, why not let me watch the kids for you, let you get a real day off.
20:57Be careful now.

21:01Man, I thought that stuff was gonna be a breeze. What was I wrong?

21:13I had to feed them twice, lunch and dinner.
21:17I played with them a little bit, but you know, I'm a big dude, so I can't really do all that with them little kids.
21:22They got endless energy.
21:25Read them some stories, watched TV with them.
21:28And other than that, I was able to clean dishes, do some laundry.
21:32So I think she was impressed. She told me she was happy about it.
21:36I'm glad I was able to be able to step up and help her out in a way that I know she would have needed because I know working all day and then coming home and taking care of her two kids and then having to watch her grandkids, I know that's a lot on her.
21:49So it was definitely a way for me to step up.
21:53I also want to make another goal for myself.
21:55I want to be able to get my license soon.
21:57You know, getting my license will solve a whole lot of problems around our house.
22:00Being able to drive will also take another load off my mom because she wouldn't have to drive me places.
22:04I could go places on my own.
22:06And also I'll be able to take and pick my sister up from school.
22:10I think I can do it. Actually, I don't think. I know I can do it. So guys, stay tuned. And hopefully you'll see some real big things from me very soon.
22:20So far, I think Caleb is doing pretty good. But I think when we met with a nutritionist and he found out all of the foods that he was going to have to give up, I could just see the wheels turning in the back of his mind of all the foods he's going to have to give up, all the stuff he's not going to be able to have anymore.
22:37I don't know, after this meeting, if he's actually going to go through with it.
22:42Honestly, we've tried every diet you could think of. At first, he made some progress. But then he hits a wall and falls away.
22:49So I hope he sees this opportunity and he takes it, even though he's going to have to give up a lot of stuff and sacrifice and don't let it all blow away.
22:58I'm really just scared for him. My biggest fear is something happening to Caleb.
23:02I'm just hoping and praying he sticks with it because I don't know how much more time he has. I really don't.

29:14And so this is something that you're going to have to try harder with your eating habit.
29:19You're going to have to make changes that are going to be sustainable because you're only 20 years old and you've got to live to be over 90 years old.
29:28So you've got 70 years to maintain healthy weight.
29:32So this is a start from here.
29:36So what you're going to do is going to be permanent changes in your eating habit.
29:40You're going to have to make sure that you're not going to fall back after surgery to own habit and gain your weight back.
29:48So you're on the right track, and I'm proud of your progress.

30:31So while he has made some good progress, it should have easily been more.
30:36So he needs to show me that he's sticking with the changes a little bit longer.
30:41Before, I'm convinced that he's ready for surgery because he has a long way to go.
30:46And it's going to get harder to lose weight, but the more he loses.

31:31Today we are doing weight loss surgery on Caleb.
31:34This operation is going to be a slive gastrectomy.
31:38It will remove 70 to 80% of his stomach.
31:41And as a result, he can eat a small amount and get full and not be hungry.
31:46This is going to help Caleb to be able to lose weight and maintain it.
31:52Eight deep deep breaths.
31:54This operation is the only tool that is going to help someone that is struggling with their weight for long term.
32:04Weight loss medication have a temporary effect, but upon discontinuing medication, weight gain is very common.
32:11Weight loss surgery is going to be a powerful tool for Caleb to maintain success for the long term.
32:22But it's not going to do the work for him.
32:24It's still going to be up to him to get to his goal weight and succeed.
32:29He's still going to have to do the hard work after the surgery to make that happen.

33:18But he had a very large stomach.
33:21And there was a lot of adhesion between his stomach and his spleen.
33:25So we had to shave off his stomach from the spleen.
33:28But we were able to handle it and complete the operation.
33:32We put a drain on him because sometimes this is associated with a tendency of staple line failure and a leak.
33:40So we had to watch him for that.
33:41I don't know.
33:43All right.
33:44We're going to let him recover 40 to 5 days before we send him home because he lives so far in California.
33:55If everything goes well, he will be able to go home.
33:58But for the next month, he's going to be on liquid diet.
34:01So no solid food for a while.
34:03And this liquid diet should lead him to lose another 40 to 50 pounds.
34:08And after that, he should be able to lose 20 to 25 pounds a month until he gets around 200 to 250 pounds.
34:16He's been doing really good.
34:18And I am very proud of him for working as hard as he has been, even though he started that really high weight.
34:24If he keeps it up, it's not going to take him long to get to his gold weight.
