A London-bound Air India plane carrying 242 souls crashed just moments after departure Thursday ... exploding in a fireball that was caught on video.

00:00The investigation is underway in Amadabad, India, after an awful disaster with Air India.
00:07A Boeing 787, just 30 seconds after taking off, and it's arguable if you'd say they even took off,
00:15never more than a few hundred feet above the ground, went back down and crashed into a,
00:23actually a medical school that's over there, so they believe that almost everyone perished aboard that plane.
00:33It was 292 people, I believe.
00:36Although at least one survivor, which is amazing if you look at that video and you see that fireball that anybody was able to survive that.
00:45Yes, that is an absolute miracle.
00:48This video is going to be critical as part of the investigation, because there have been, you know,
00:54aviation experts who looked at this and said that the wings of the plane were never configured the way they should have been for takeoff.
01:02They were configured more for a landing and never changed out of that.
01:06So something either went wrong, I guess it could be pilot error.
01:10They just don't know yet.
01:12They are still, you know, sorting through all of the debris and certainly trying to find the black boxes and find out what happened here.
01:21The thing is that the Dreamliner, this is the first Boeing, this is their big international plane.
01:29This is the one that if you're flying to Europe, you fly to Asia from the U.S. and you're on a Boeing.
01:34It's probably a 787 and they've never had a tragedy like this.
01:40They've never had a fatality on the Dreamliner, but there were some warning signs, including right here on TMZ Live.
01:46Yes.
01:47You'll remember just over a year ago when there were all the questions about Boeing's 737 Max.
01:53There were a few whistleblowers who spoke out.
01:56John Barnett was one of them.
01:58He was on TMZ Live in January of 2024.
02:03And we had him on to talk about the 737.
02:06Listen to what he said about the Dreamliner.
02:09Back in 2012, Boeing started removing inspection operations off their jobs.
02:16So it left the mechanics to buy off their own work.
02:18My concerns are with the 737 and the 787 because those programs have really embraced the theory that quality is overhead and non-value added.
02:32So those two programs have really put a strong effort into removing quality from the process.
02:40So we don't know exactly what happened here.
02:43And so he may be right.
02:46He may not be right, but it's really interesting the way that he talked about that as a potential preview.
02:52Yeah, we have to stress that.
02:53I mean, the investigation here is literally just getting underway.
02:56This just happened.
02:58So as you said, that picture is interesting about the configuration of the wings, but we'll have to see.
03:02There's a lot of explanations for that.
03:04It could be a pilot.
03:05It could be something mechanical on the plane.
03:07Obviously, the name Boeing raises the specter of what happened in 2024 with numerous of their aircrafts in the news for the wrong reasons,
03:14for mechanical problems and quality problems that you heard Barnett talking about there.
03:19So we'll have to see.
03:20It's not been connected yet, but it does raise a lot of questions.
03:23If you remember, just another interesting part of John Barnett's story, that two months after we did that interview with him, he died of suicide.
03:33That is what the official said when they found his body.
03:37His family filed a wrongful death lawsuit against Boeing, which was actually just settled last month.
03:45Boeing, by the way, their CEO, came out today and did say that they are, obviously, they're concerned for all the families of the victims today
03:56and that they are going to be doing everything they can to assist them and to assist in the investigation in India as to what caused this crash.
04:04And I'm curious to hear from the survivor.
04:06Yes.
04:07That must have been like two.
04:08One guy who is, they believe, to be the only survivor from the plane.
04:14What a story that is, too.
04:15Yes.
04:16Wow.

04:19I just want to first off, you know, say my heart goes out to all the victims and the families.
04:26But just that one guy that did survive, I definitely would love to hear his story as well,
04:32because that's a blessing in itself, just, you know, being in a situation like that, looking at the video,
04:38and then there's a survivor left from that.
04:41It's just like, wow, you know, he was really blessed to be still here on this earth.
04:46Talk about survivor's guilt, too, and everything else.
04:49Right.
04:50Yeah.
04:52Well, we will see where that investigation goes.
