00:00 Hello there, welcome to the final installation of putting Dr G on the spot in the month of

00:05 April, talking about various conditions that result in sexual dysfunction related to prostate.

00:12 On this occasion, we're going to focus on one condition called Prostadenia.

00:17 Prostadenia itself just means prostate painful.

00:28 What it means is that when somebody actually has ejaculation and it is painful.

00:33 Why would that be the case?

00:35 In fact, we actually don't know the prevalence of Prostadenia.

00:40 Most of the time, we actually don't know the etiology of Prostadenia.

00:44 If we dissect all the people who presented to urologists with Prostadenia, when we realize

00:50 that some of them is caused by prostate infection called prostatitis, some of them is caused

00:57 by ejaculative disorders such as blockage of the prostatic ducts and some of them is

01:04 actually have no cause at all.

01:06 We call idiopathic Prostadenia.

01:09 So when it comes to prostatitis, there's actually infective and non-infective changes.

01:17 For the infective changes, it actually can be caused by bacteria such as E. coli.

01:23 For non-infective changes, in fact, when we culture the semen, there's no infection at

01:28 all.

01:29 So under all these conditions, we call it chronic pelvic pain syndrome.

01:35 Of course, if the pain itself is very mild, which most cases are, it's not problematic.

01:42 However, when the pain itself is very significant, causing erectile dysfunction and also premature

01:51 ejaculation and also distress for avoiding of sexual activity, then it becomes problematic.

01:59 When we assess about 11,185 patients to look at the causes of Prostadenia and also what

02:09 resulted in sexual dysfunction, we realize that 30% of men who has got Prostadenia will

02:17 have resultant erectile dysfunction.

02:21 And about 40 to 50% of these men may even have premature ejaculation.

02:26 Therefore, it itself may lead to confidence level problems and also avoidance of sexual

02:33 activity.

02:34 The good news is that the vast majority of these patients, the problem may be self-limiting

02:40 and it disappears with time.

02:42 For a minority of patients, this may require multiple antibiotics or even medications such

02:49 as alpha blockers.

02:51 Alpha blockers itself may be useful in order to relax the prostate and reduce that infections

02:58 and also inflammation.

02:59 However, it has a side effect which we've discussed about, which actually causing retrograde

03:05 ejaculation and might result in other form of frustration in sexual dysfunction.

03:11 So how time flies, we are venturing into the month of May and in the month of May, we're

03:16 going to be discussing everything to do with scrotum.

03:19 What we're going to discuss actually is scrotal pain not related to scrotum but scrotal pain

03:24 related to kidney stone.
