00:20 For our first question,

00:22 "Doctor, can a woman with child already have PCOS or polycystic ovarian syndrome?"

00:28 "What is the reason for this?"

00:36 PCOS is a condition that occurs in reproductive-age women,

00:40 whether they are pregnant or not.

00:43 PCOS is polycystic ovarian syndrome,

00:46 where there are many manifestations.

00:50 There is no period, sometimes menstrual bleeding is heavy,

00:54 there are pimples, and abnormal pigmentation in other parts of the body.

00:59 This is due to hormonal imbalance.

01:02 The estrogen level can be high, the progesterone level can be low,

01:05 or there is insulin resistance in a woman, like diabetes mellitus.

01:10 Or PCOS can also occur in a woman if she has an abrupt weight gain or abrupt weight loss.

01:19 "Is it natural for a woman to have menstruation for 3 weeks?"

01:28 A normal menstruation can last for 3 to 7 days.

01:32 The period from the first day of the last menstruation to the first day of the next menstruation

01:37 is around 28 days, plus or minus 7 days.

01:40 This means that a woman can have menstruation within 21 days or 35 days.

01:46 So, in one month, she can have 2 menstrual cycles,

01:50 or there is a month where she cannot have menstruation,

01:54 and then the following week of the next month, she can have menstruation.

01:57 If this exceeds 35 days, or a woman has menstruation for less than 21 days,

02:03 this is not normal.

02:05 This is what we call abnormal uterine bleeding.

02:08 If a woman's bleeding exceeds 7 days,

02:12 or if it is more severe than her previous cycles,

02:16 this is what we call abnormal uterine bleeding.

02:30 Amenorrhea is the loss of a woman's period.

02:34 We call it primary amenorrhea and secondary amenorrhea.

02:38 Primary amenorrhea is when a woman reaches 15 years of age and does not have periods.

02:46 Secondary amenorrhea is when a woman has periods before,

02:51 and then for the 3 succeeding months, she does not have periods.

02:57 There are many causes of primary amenorrhea.

03:00 It can be hormonal, genetic, or chromosomal aberrations like Turner's syndrome.

03:06 The secondary amenorrhea causes are the following.

03:09 It can be hormonal imbalance,

03:11 or it can be hormonal imbalance in other parts of the endocrine system of our body

03:18 like thyroid glands or adrenal glands.

03:22 If a woman has hyperthyroidism or adrenal insufficiency,

03:26 or if a woman undergoes dialysis because of chronic renal failure,

03:30 amenorrhea can also occur.

03:32 Malignancies or infections are the other causes of amenorrhea.

03:37 And on to our next question again, Doc Q.

03:40 Why is it that her 18-year-old son has not yet had a menstruation?

03:46 If your son is 18 years old and has not yet had a menstruation,

03:50 this is what we call primary amenorrhea.

03:53 And it needs to be examined to find out if the problem is anatomic or hormonal.

04:01 Because if it is anatomic, it is difficult to correct if he does not have a matrix

04:05 or if his ovarian is underdeveloped and under functioning or hypofunctioning.

04:11 Or it can also be labial agglutination where the labia of a child is closed.
