In a major naval accident, INS Sindhurakshak -- a kilo class submarine that had undergone a major refit at Russia's Zvezdochka shipyard -- submerged at her berth at the Naval Dockyard in Mumbai after a major explosion ripped past the submarine late Tuesday night.
