Blepharoptosis or Ptosis or drooping upper eyelid is a common eyelid disease. If many people wonder why you look sleepy, or if one eye looks smaller than the other, you probably have upper eyelid ptosis. Ptosis describes an abnormally low upper eyelid, resulting in a very sleepy eyed look. While in the initial stages, you may just look less alert, in the later stage this may lead to tired eyes, more watering from eyes and even obstruction to vision as the droopy eyelid prevents you from seeing.

Ptosis can be classified in several ways. The common way is to classify as congenital (Ptosis in children) or acquired (Ptosis in adults) and as (1) aponeurotic, (2) myogenic, (3) neurogenic, or (4) mechanical. This is important as even though adults as well as children can have this eyelid abnormality, treating this can require completely different surgical treatment procedures and can have completely different results, hence diagnosing the aetiology (cause of ptosis) is important.

Ptosis surgery is amongst the most unforgiving of aesthetic and cosmetic eyelid surgery procedures, as everyone, I repeat everyone, can tell if one eyelid is even half a mm over or under the other. Ptosis surgery requires much skill and is best performed by an expert in facial plastic and cosmetic, oculoplastic and eyelid surgery, since performing surgery is just one part of the problem, but more importantly given the dynamics of the eyelid and the critical role it plays in protecting the eye from dust and injury, the after-care can be substantially more important than even the surgery itself.

All in all, ptosis surgery is complicated and needs considerable time, effort and dedication to master the skills. In this video, Dr. Debraj Shome, one of the best oculoplastic surgeons in the world (currently based in Mumbai, India), demonstrates three different types of ptosis surgeries, in patients with three different aetiologies/ causes for the eyelid ptosis, resulting in excellent cosmetic outcomes in all three patients.

This patient was treated at The Esthetic Clinic, one of India's top most centers (http://www.theestheticclinic.com). The surgery is performed by and licensed to Dr. Debraj Shome (http://www.facialplasticsurgeonindia.com), Director of The Esthetic Clinic, Mumbai, India. Dr. Debraj Shome has dual training in ophthalmology and in plastic surgery and hence is best suited to treat complex eyelid problems like ptosis, baggy eyelids, lid retraction, entropion, ectropion and eyelid skin cancers.
