00:00Today we are going to talk about a topic which is much neglected.
00:06Everybody talks about how to improve success and how to deal with a specific situation.
00:12But then at the end of the day there is a subset of couples where success doesn't come.
00:17And they are left in an emotional lurch.
00:21So how do they deal with it and what to do further to improve their prognosis
00:26and they help them in achieving the goal that they set out to.
00:31So first of all I would say that one should have realistic expectation when they start an IVF cycle.
00:38Because no matter how much ever we try to make it successful it's a scientific process.
00:45So it can't be a 100% successful procedure today.
00:49Because the process of reproduction being very very complex.
00:52We are still as the entire IVF fraternity worldwide.
00:56We are in the process of learning it, discovering more about it.
01:00And trying to solve the problems which can come, which can interfere with reproduction.
01:06So the emotional part has to be dealt with patience and realistic expectations.
01:13If needed please go for counselling and understand what exactly is the problem.
01:19And what are your chances, what are the options available.
01:22So once your cycle turns negative, once you are out of the initial shock.
01:27Please visit your doctor.
01:29Discuss the details about what happened exactly during your last IVF cycle.
01:34And what looks like the probable cause.
01:38It is very difficult to point out the exact cause why an IVF cycle didn't work.
01:42But your doctor will be able to give you some idea.
01:45Because of the progressive learning that we get while we conduct a couple's IVF cycle.
01:51We understand that this could be the problematic area.
01:54And if there is a next time then we can work more on it.
01:58So for example if you have done a fresh cycle and you still have frozen embryos remaining with the clinic.
02:04You can opt for a frozen embryo transfer cycle as your next attempt.
02:09If you have frozen embryos.
02:10In that case if endometrial development had not been optimized or some new findings had come.
02:16Then that has to be optimized.
02:18If required a histoscopy can be done to get a better idea of the uterine cavity.
02:22If required a biopsy can be done.
02:25Sometimes when there are very subtle issues.
02:28There are some experimental tests like error that also can be done.
02:32Then say for example if the couple is in the higher age bracket.
02:37Or if there is a history of miscarriage.
02:39If the indication is there.
02:41Then some more optimization can be done by testing the embryo.
02:45Whether it is normal genetically.
02:47For example aneuploidy testing.
02:49Pre-implantation genetic diagnosis is the term for that.
02:52And time the transfer better.
02:55Then at the same time there are some therapies which are not proven very clearly.
03:00But they are considered a good practice.
03:03Say for example if there is a recurrent implantation failure.
03:06Then sometimes there is a use of heparin injection.
03:09No molecular with heparin or aspirin.
03:12Which are known to help in implantation.
03:14There are more therapies.
03:16But because convincing data is lacking.
03:19So some people are using it.
03:22But that cannot be given as a blanket treatment for all couples.
03:27So essentially it is about finding out what are the probable causes.
03:31What learning has come from the last cycle.
03:34And then how can we optimize it.
03:36Say for example if getting the egg number or quality was the issue.
03:41And if this appears to be a recurrent problem.
03:44If the lady is on the higher age bracket.
03:46Then consider maybe using donor eggs.
03:50Then if there is a say for example some problem with the uterine cavity.
03:56That has to be corrected.
03:57At the same time while you are undergoing your next cycle of fertility.
04:01All the time you have to be in touch with your counsellor.
04:07And you need to have very good support system.
04:10Both in the family as well as in the clinic.
04:12So any clinic worth it sort.
04:14Any IVF clinic will have that support system available.
04:17Which is just a phone call away.
04:19Whenever you feel stressed out.
04:21There are lots of things to be done in the embryology lab.
04:24Apart from trying to create the best possible embryo for the couple.
04:27There are some experimental therapies for example.
04:31Co-culture.
04:32So lots of other things are there.
04:34So there some improvisation could be possible.
04:37So all of it can help a couple.
04:39Which has not conceived in their first attempt of IVF.
04:42So in subsequent attempt we can of course try and increase their chances.
