So quite aggressive, the table might actually break.
00:12
Are we all ready if I start teaching now?
00:15
Okay, first thing's important, you need to be able to recognise what's going on.
00:22
So the key thing with this is that you have an unresponsive casualty.
00:26
So if you're going to go out and you're going to shout their name,
00:28
if you're going to give them something to tap on the shoulder,
00:30
you're not going to get any response.
00:32
The other thing that you're not going to have is you're going to have an unresponsive,
00:36
not breathing casualty, or someone that's not breathing normally.
00:39
So sometimes when people go into cardiac arrest initially,
00:42
they have some very abnormal slow breaths, like push out of water.
00:46
That's not normal, that's cardiac arrest, non-breathing, not breathing normally,
00:51
and unresponsive as soon as cardiac arrest.
00:54
So once you've done that, you're going to go,
00:56
Hello, can you hear me? No, no response, okay.
00:59
I'm recognising a cardiac arrest, I need help.
01:02
I can't do this on my own, so I'm going to call for help.
01:04
We're going to call 999, put our phone out on speakerphone,
01:08
because we all carry our phones in our pockets these days,
01:10
so get that out on speakerphone.
01:12
The call handle will stay online and we'll talk you through everything I'll show you,
01:15
but practice when this is the first thing to do when you're ready for this.
01:18
Okay, so we're going to lock our hands together,
01:21
and you're going to use the heel of your hand,
01:24
and you're going to direct that pressure on the lower half of the breastbone.
01:28
You can say sternum because this is Scottish,
01:30
but everyone knows what the lower half of the sternum is these days, okay?
01:33
So, lower half of the sternum, hands together,
01:36
and you're going to push down, you want to push down the chest about a third of the depth,
01:40
a rate of 100 to 120 per minute.
01:43
Okay, so this little device here is going to give you some feedback.
01:46
On you go.
01:49
Okay, is that perfect?
01:52
So you can see that you're lighting up white lights there,
01:54
and that is in the dream zone, so you're in the right rate.
01:57
It's really important that you are going at the right rate,
02:00
and you're allowing enough time for your hands to come up.
02:03
So what your pecs are doing there is squeezing the heart against the backbone,
02:06
that's emptying the blood out.
02:08
When you lift your hands up, the heart's being filled, and you're going down.
02:10
So you're artificial circulation going on there.
02:13
You've called for your ambulance, your call handle's on the phone,
02:16
and hopefully help is going to arrive, bringing one of these devices.
02:20
Okay, so the key is we need to minimise interruptions to compassion,
02:24
so I need you to keep going.
02:25
Okay.
02:26
I'm going to work round you.
02:27
How am I supposed to know what 120 beats per minute is?
02:29
Well, it's roughly two per second, so one and two and one and two.
02:34
Got you.
02:35
Okay, so if you don't have one of these devices there, you can do that.
02:39
Okay, so I'm going to get this out.
02:42
All of these work in roughly the same way.
02:44
We'll give you little instructions along the bottom.
02:46
So we need to turn the device on.
02:49
You keep going, you're doing a fabulous job there.
02:51
I'll do that.
02:53
Staying alive.
02:54
Yeah, I know, it's Bee Gees, isn't it?
02:56
That's it.
02:57
Oh, so that's what I was kind of fishing for?
02:59
Yeah.
03:00
I'm not giving you that one.
03:01
That's a very nice APM.
03:02
Okay, go.
03:03
No, you need to keep doing that.
03:04
I need to work round you.
03:05
Okay.
03:06
I'm going to put these pads on.
03:08
Just show me roughly where I'm going here.
03:12
Okay, so this will have to put these pads on in the wrong order,
03:15
as long as they're in the correct place.
03:17
Okay, so you can put this in.
03:23
Need to go a bit faster.
03:24
That's it.
03:27
Okay, so this is analyzed.
03:28
I need you to come off the chest just now.
03:30
I'm just going to listen out for what you need to do.
03:35
Okay.
03:37
So we need to make sure nobody's touching the patient while we're doing this.
03:40
This will charge up.
03:43
Okay, I'm going to push this button,
03:45
and you need to go back on the chest.
03:48
Now, you just go back on the chest.
03:49
We want to minimize any interruptions to the CPR.
03:52
This is what's buying us time and helping save a life.
03:55
So have you just shot them?
03:56
I've just shot them.
03:57
So even if the shock has been successful,
03:59
it's going to take a little bit of time before everything comes back online,
04:02
so do carry on doing your CPR until you're physically exhausted,
04:06
in which case someone will hopefully come and take over from you.
04:09
Help arrives, or you can see physical signs that the person is returning to life.
04:15
This will analyze every two minutes.
04:17
There's two minutes of CPR,
04:18
and two minutes feels like a really, really long time,
04:20
so it's important that someone else can take over
04:22
and answer those two questions for you.
04:24
And again, you guys, if the gentlemen want to come and test out what they are,
04:27
then you're very welcome.
04:29
So there's no mouth in?
04:31
No, we're taking that out.
04:32
We know the important bits for survival is that early recognition and call for help,
04:37
good quality CPR, minimizing interruptions,
04:40
and getting this back on the shocking as quickly as you can,
04:43
ideally within that 35 minutes after the person arrives.
04:46
This is a lot quicker than I thought it would be.
04:48
It's also a lot harder.
04:50
It's getting a little tricky.
04:52
It's getting tricky? Do you need me to take over?
