01:44An investigation has reportedly revealed several security failures,
01:48including that a Secret Service radio room had no way of receiving real-time alerts from local police.
01:54The agency's head, Kimberly Cheetle, resigned amid the ensuing scrutiny,
01:58while at least five agents were placed on administrative leave.
