Two rail companies fined $525,000 over fatal derailment

Two rail operators have been fined more than 500-thousand dollars over a deadly train crash on the outskirts of Melbourne four years ago. The Melbourne magistrate's court heard the tragedy could easily have been prevented if basic safety measures had been in place.
