This could happen when a wrong value fuse is inserted in a fuseboard. In this case I used a standard appliance lead ( ex coffee machine ) rated at 7.5 Amps, 3/4 mm² on a 10 Amp socket. Current passed through the lead was around 63 Amps which is a commonly used Amperage of a mains fuse in New Zealand. Within a short period the lead overheats, emits toxic smoke and starts to burn.
