00:47It actually feels like I have a boob groin on my back.
00:51If it is under the muscle, I really don't remove those.
00:54That is heavy.
00:56I can imagine the weight of that.
00:57Multiple little lumps to it.
01:01Lipoma's got a lipoma.
01:04What do we have in here?
01:05Try to wheeze it out.

01:10I'm Allie, and I am 46 years old, and I have this annoying pump on my face.
01:33It's about the size of a piece of bubble gum.
01:35It's a little squishy, a little hard.
01:39It feels like thick mashed potatoes.
01:44The varicose veins that you get on your legs, there's, like, a couple of those that run through there.
01:50I feel like it's one of the first things people notice about me.
01:54There was this one time I was at a bar, and this guy came up to me and just said, you know, you'd look a lot prettier without that thing on your face.
02:02This bump has caused me to go through life with my head down.
02:05I am a pagan individual.
02:13Paganism is like a nature worship religion.
02:17I briefly thought that I could look up a spell or anything else to make this go away.
02:23Spells are just, like, tactile-driven prayer.
02:26And turns out there aren't any spells for removing a lump on the side of your face.
02:36I've been living with this monster on my face for five years now.
02:41This bump started off as a little tiny pimple, but it didn't pop like a pimple normally does.
02:47I went to the doctor, and she sent me to this vein surgeon.
02:54They're like, no, we don't handle that.
02:57Then gave me another referral for a different place, and they're like, no, we don't handle that.
03:03After a while, it was just like, well, I guess that's too hard to take care of, and kind of got scratched off the list.
03:10So, I find that extraordinarily frustrating, because the bump kind of feels like it's sucking the joy out of my life.

03:28Cody and I have been together 11 years, and we have been married 7 years.
03:33He looks nice.

03:47We were in a square, that's what we called it one time, where he was dating another lady, and I was dating her partner.

04:05And so, polyamory is just kind of a major extension off of that.

08:33A bump this prominent on your eye can really destroy your confidence in so many ways.

08:56It definitely is a cyst.
08:57I try not to squeeze it too much.
08:58It feels pretty tense, like it's built.

09:01And the main things that I think about, obviously, is it's around your eye area.
09:06And I think it's very important to get this removed.
09:08Because I think that if it gets inflamed or infected, it can cause a lot of problems.
09:11And you don't want to have this major infection or swelling in the central part of your face.
09:16And the other thing I'm thinking about is how to remove this.
09:20Making use of your eyebrow and hiding the scar line within your eyebrow.
09:24You know, I just want it to look as best as it can.

10:05You know, this skin is the thinnest skin on your entire body.
10:08It's less than a millimeter thick.
10:10Any kind of quick movements could do something very drastic and irreversible, like puncturing an eyeball or damaging the muscles to open the eye.

10:22So make sure that you don't feel anything other than my touching you, okay?

11:07This strip of skin is there because I'm going to remove that strip of skin with the cyst so that when I close the area up, there's less laxity in the skin.

14:38They're not painful, but they're there and they're kind of annoying.
14:43Like Walt Disney would come in here and take his lunches.

14:48Every time we come to California, we try to take in a slice of film history.
14:53This diner's been featured in a lot of films, Once Upon a Time in Hollywood and Twin Peaks.
14:58We'll go and try to recreate some scenes.

15:27The spa, they itch all the time.
15:30So it's irritable.
15:30I mean, just think of like constantly having to itch everywhere.
15:34And I'll end up with flakes that just fall off the body.
15:37If I continually scratch like the same place, it'll break open.
15:41It'll start bleeding.
15:42I constantly have to pat you because I see that.
15:46I mean pieces of myself all over the world.
15:50My skin condition developed when I was just a child.
15:53I really have no recollection of life without it.
15:57My parents have it a little bit, but not as severe as I do.
16:00I was told it was psoriasis.
16:02It was incurable.
16:05My mom tried every old wives remedy creams that just smelled disgusting, made me smell disgusting.
16:11She used to put some creams on my legs and wrap my legs in like saran wrap.
16:16And I was just miserable.
16:18I just wanted to be a normal kid, and I felt like I couldn't do that.
16:23And then add to it the little lipomas that came along too here.
16:28And I can feel it against the chair.
16:312012, I went to see a general practitioner about the bumps on my arms, and he said that was a lipoma.
16:38I asked if I could get them removed, but he didn't really want to do that.
16:42I've been very skeptical about what doctors can do for psoriasis because I've always been told my whole life there's no real cure for the disease.
16:48I was tired of not finding a solution.
16:51I gave up on my skin a long time ago.
16:55Like, don't you want to be comfortable, confident, you know?
16:58Yeah.
16:59I also feel stuck.
17:02And I really want to have a kid.
17:05And I'm not getting any younger.
17:06I know.
17:06We've been delaying having kids for a while, and we want to start a family.
17:13I want it to be sorted out before having children, so I urged Andrew to look at this again.
17:21It's hereditary, so I definitely had kids I wouldn't want to pass it on to them.
17:25That weighs on my mind.
17:29If I was to be a father, I'd want to be the best dad possible, but I don't want to have them deal with the same things that I've dealt with.

18:41So, I've had psoriasis pretty much my whole life.
18:46It's just on my legs, my arms.
18:49It's on my chest and stomach, my back.
18:52Since you're about what age, would you say?
18:55Eight or nine years old.
18:56I can't remember how old I was.
18:58That's pretty dumb.
18:58So, you haven't been clear in quite a while.
19:00No, I mean, this is...
19:01You've just been living like this for a while.
19:02This is my life, yeah.
19:03Yeah.
19:04I was told this disease is hereditary.
19:07Psoriasis can run in families, but it's not destined.
19:10It's not a definite that you will have kids that inherit this.
19:14It is not life-threatening, but it is a chronic condition that can be a pain, as you know.
19:18But there are really wonderful things that can control it these days.
19:21So, I have that, and then I have these lipomas.
19:25Bones on you?
19:25They've been called lipomas, yeah.
19:28Here, just relax that.
19:29I'm just going to feel like I can pinch it.
19:31Where else do you have another one?
19:33Yeah, there's one on my back that's about the same size.
19:37Okay.
19:38Psoriasis and lipomas.
19:39I mean, Andrew's got the double whammy of unlucky dermis.
19:43There is no relation between these two conditions.
19:45It's really coincidence that he has both.
19:47Psoriasis is not life-threatening, but really kind of gives you this flaky, embarrassing skin.
19:51And then she's got lipomas, which are also benign.
19:54These bumps that are really unnerving and really embarrassing on the skin as well.
19:58He's a dermatologist's dream.
20:00When was the last treatment that you had?
20:03It was back around the pandemic.
20:05Okay.
20:05I had a dermatologist that prescribed your primalast.
20:08Okay.
20:09And that...
20:10That's a pill that you take, right?
20:11It's a pill, yeah.
20:12It cleared up a little bit of my arms.
20:15But not so much your torso and your legs, huh?
20:18No, nothing else so much.
20:20Okay.
20:21Okay.
20:21That is one of the treatment options for psoriasis.
20:25It's in this whole category that we call biologic kind of treatment options.
20:30Did that clear you up?
20:31It didn't clear up everything.
20:33Yeah.
20:34Okay.
20:34I actually think that an injectable, the newer ones, are safer.
20:39However, they are more specific, so I think they're going to work much better.
20:44Not all biologic medications are created equal, and they all have a different kind of function.
20:49And some people respond better to one versus the other.
20:52And there's a lot of newer, more modern, injectable biologics out there, which seem to work even
20:59better at psoriasis.
21:01And so let's try one of those.
21:02You have some lipomas.
21:04Yeah.
21:04The thing I'm concerned about is more that, you know, doing a surgery over your area of
21:09psoriasis.
21:10I feel it's not proper medicine for me to do an incision right over an area that's pretty
21:16involved.
21:17The two large ones are the ones behind your right arm.
21:20There's a pretty good amount of psoriasis over this.
21:23So let's get our psoriasis in shape first, you know, cleared up before we were to address
21:28that.
21:29All right.
21:29The one on your back has less psoriasis overlying it.
21:32So I think we can get that one out safely.
21:34Let's get rid of the one on your flank.
21:36I wish I could get all of his lipomas, but I think I can only remove one today because
21:40the others have psoriasis overlying them.
21:43And there's more risk of infection in that area.
21:47There's risk of cabnerization, which means that the psoriasis can worsen if you kind of
21:51traumatize it on the surface of your skin.
21:53So the plan is let's try to get you on a new biologic medication, a newer class of them
21:59that I think are more effective.
22:00And let's take off that bump.

22:12What if there's a blood vessel that gets cut open or what if it doesn't go the way that
22:16it's supposed to?

22:42oh did i it's ticklish i think andrew's lipoma is laughing at me it makes a funny little sound

23:28comes out in multiples like this it really wants to tear easily and that's trouble oh man

24:04leave any of it in there because if it does tear apart and it detaches part of it will just suck
24:09back into the body this one likes to be in a cluster i suppose it's growing in more of a cluster

24:22okay i got it wow i think we got it all there we brought sexist back back that was quite a stubborn
24:33lipoma but it actually ended up being pretty cool for a very sweet guy and i'm gonna stitch
24:37him up and i'm gonna start him on a new modern injectable biologic that i think is gonna clear
24:44him up and hopefully it'll remind him to see his dermatologist more regularly
24:48we remove that if you want i don't know if you could reach back with your hand and feel where that
24:54is oh yeah that is numb too but also flat huh yes it is after trying so many different things i
25:03completely gave up so to remove the lump on my back it feels like we won the lottery and it is a huge
25:11relief to have that gone so we're gonna do an injection of guselcomab i hope that this new
25:18medication being a newer sort of version of it is gonna work better and it's gonna clear you up instead
25:24of just sort of letting it smolder which wasn't so great i look forward to seeing your skin all
25:29cleared up thank you so much okay nice to meet you years ago i took a biologic that really didn't clear
25:35me up that much so i'm very excited right now that i have an open path toward clear skin i feel really
25:41grateful toward dr b she helped us out she stepped in and when nobody else was able to help us she came
25:46she came to the rescue i feel hopeful and i feel like there's nothing holding us back now from

26:28it's huge and it's painful this bump has grown from a lemon into a like a softball and it feels
26:40very squishy like i can't move it around it is just uncomfortable it actually feels like i have a boob

28:21i thought it was something cancerous so i got really scared i was like why did you wait so long to tell

29:47bit more surgical experience i really want to show dr lindsay how i evaluate a lipoma like this and
29:53decide whether she's indeed a candidate for surgery today we have like we classify it as a big lipoma
30:00this is a big one that a lot of dermatologists wouldn't do i really want to pass the knowledge
30:06on to people that i also personally trust that i think it can do the similar kinds of things and i feel
30:12like really care about their patients okay okay good well let's go see the patient okay i'm excited
30:17and i'm ready for that big one this is actually my first big lipoma excision and i'm just hoping

31:00i try to evaluate whether something is under the muscle because if it is under the muscle i really
31:05don't remove those oh okay can you actually make movements to like shift it and we see if it stiffens
31:11up i do think this is a lipoma sometimes lipomas can reside under the muscle and that really is when
31:18it becomes more complicated to remove it this is a great situation for dr lindsay to know how i try
31:24to do all the things that i can to get the most information before i proceed with surgery so it
31:29doesn't really just stiffens up enough so you could still push on it if it was like hard that to me
31:34makes me think because then the muscle because it'd be tense right but yeah it does feel like
31:39lipoma i should tell you that it's not life-threatening so if you were to coexist with it it is
31:44possible but you might not like your coexistence with it yes i want to go on yes okay we're gonna
31:50go start getting set up here okay in the meantime but let us know if you have any questions okay okay
31:54thank you so much thank you i am feeling nervous my crazy thoughts thinking like i'll bleed out or
32:02something and they have to stop the surgery yes it would really suck if dr lee cannot remove this
32:10it's just so traumatic to me i just want it gone
32:21okay you got your gown on thank you for that so we're gonna start putting some numbing there and
32:26letting it sit for a little bit i can wait in the room oh yeah in the waiting room first i'm gonna
32:34numb christina with tumescent anesthesia and by doing that i'm filling the area with fluid creating
32:40a cushion and lifting this lipoma away from important structures deep underneath the skin if
32:46you pull it out you want to clamp it first i really want dr lindsay to see the things that i do
32:52to prep for a surgery like this i mark out where i see veins on the surface of the skin if i know
32:57where the veins are and i cut in a certain area and there's a little bit of bleeding i know it's
33:02because there's a vein there okay just make sure i'm not hurting you at all okay okay
33:11doing great and i know it's deeper under there the actual lipoma
33:17you're doing awesome a little buzzing sound okay nothing should hurt okay i just preemptively kind
33:26of buzz things because i like to keep things clean and then i really use my fingers a lot you can
33:31actually take a feel you just pull your wristband no i don't know because it might be a vessel you
33:40don't just try to yank it this is a really important lesson for dr lindsay to see i don't blindly
33:46cut or pull on things some of those fibrous bands can be blood vessels they can be nerves and so you
33:52just really want to be very precise in all your movements so you can literally just pull wow that's
34:00what i try to do so that i don't have to traumatize things the negative is that it is that friable so
34:06if i'm too aggressive and try to pull it it'll just tear so i'm trying to just ease it out with just enough
34:11pressure you can feel how it extends into different spaces like different tunnels under there okay
34:17try to squeeze it out it's not going to squirt you okay yeah okay but it is coming out a little bit yeah
34:26oh yeah i should have done that there you go i did that but there's still a little
34:32the larger lipomas is just a larger risk of bleeding right well i mean it's just because of the size
34:38and more blood vessels dr lindsay is so cute because she's so scared of squirts but i think
34:43this is a really good case because she can see that when a lipoma gets pretty big it can get
34:49multi-lobulated and has these sort of tentacles that dive down in the skin in all different directions
34:56oh wow is it still there yeah i'm trying not to unleash it hold on there's a lesson here
35:08we have to stop the bleeding right here
35:10i'm here with christina and i'm showing dr lindsay how to gently remove a lipoma and i think the key
35:33here is slow and steady you don't want a big mess of blood so i use a little surgical tie to kind of
35:39tie off that blood vessel thank you so is it a boy or a girl uh do i have twins because it's a
35:47little bit of trouble a little trouble so you basically tied off the the vessel right yeah and
35:53i just try not to be aggressive really deep i just try to you can see i'm kind of like i feel like i'm
35:59kind of dancing with it
36:08oh wow i was able to safely get out all the lipoma and certainly control any kind of bleeders along the
36:14way and you know when you start in dermatology you learn all about all skin conditions but there's a
36:19big difference between learning and actually doing it so i'm glad that i was able to show dr lindsay and
36:25she did a great job i always line up the edges first because if you don't do that you're gonna
36:31it's hard it's easier for you to be offline i was taught to start in the middle and like a rule of
36:36right right a lot of people do that but sometimes if you do that you won't have it lined up at the
36:41edges it's a neat trick pulling out the lipoma today was really satisfying so just from this
36:49one procedure i've learned so many things that i can definitely take with me in my own practice
36:55this is literally the biggest lipoma i've ever excised i'm gonna let you reach back with your
37:00hand and feel this it's the towel over it okay do you want to reach back and go no but it's not there
37:06anymore are you crying oh oh that's really it looks so good i'm so proud of you oh are these tears of
37:19happiness yes okay i just want to make sure i'm so happy thank you all i've lived with this bump for
37:25five years and the stress was overwhelming and now that it's gone i am so relieved here's your friend
37:37so this was the bottom that kind of extended kind of like where it made your look bigger there okay
37:44oh oh my goodness it did look like chicken i think you're gonna look great awesome thank you so much
37:52so nice again of course thank you it's like a roller coaster i was nervous and now i'm just
37:57excited and so happy oh my goodness it's just puffy because of numbing now all the shirts that i
38:07borrowed i'm gonna have to unfortunately probably get back the first thing i want to do is dress up
38:15something cute go out with nicole hit the beach and get a nice tan going on both shoulders and expose them
38:22to the fullest it's been three months since i saw christina and her back looks flat and beautiful
38:34it feels great now that the huge bump is gone and that this was not something serious the scar from
38:41that big old bump has healed up so nicely and christina is finally back to feeling like her old self
38:46now she can dress how she wants and keep up with her daughter nicole we are spending a little too
38:52much money probably a family that parties together stays together cheers cheers guys
39:02it's been three months since i saw andrew and his lovely wife gift that cluster of grapes lipoma is
39:08gone this scar has healed up so nicely and andrew's psoriasis has cleared up beautifully
39:16i don't have flakes on my shoulders or anything i've completely cleared up night and day difference
39:25his dry flaky red patches that covered his body are gone and his skin is spotless
39:33honey you ready to watch the movie now andrew and gift are ready to move forward and take their next
39:38steps in life they are so cute i'm rooting for you guys
39:51it's been two months since dr lee removed my big pump and i am feeling amazing
39:57i feel like i i'm beautiful again the scar is hidden in my eyebrow and i actually like
40:09looking in the mirror now and what's best is i have my confidence back
40:18since the bump has been gone cody and i have been getting out more
40:22more and we're definitely looking for our next potential partners i'm in a better place to have
40:29a polyamorous lifestyle now i'm cody this is ally nice to meet you i love the dragons they're great
