00:00Hello everybody, I am Dr. Shalab Bansal from Tesla Imaging and Diagnostic Center, Merit.
00:05Today we will tell you how to do the PICTIL insertion.
00:08First of all, we will anesthetize the local 2% of the skin and subcutaneous fat.
00:18After giving local anesthetic, we have already prepared the part.
00:23Now we have taken the 18 gauge spinal needle.
00:26We are inserting it into the core of the abscess under the help of the ultrasound.
00:32So after the ultrasound of the abscess, we will confirm the middle of the abscess.
00:37After that, we will also know the direction of which we have to go.
00:41So once we have ascertained the direction that we are into the core of the abscess.
00:48So we are going to then take the scalpel and put the incision on the skin and subcutaneous fat in the mid-axillary line along the intercostal space.
01:01We are going to take out the PICTIL catheter.
01:03So now, avoiding the injury to the diaphragm and lung,
01:20as we can see on the ultrasound, we are going straight into the core of the abscess.
01:36As we can see on the ultrasound, the PICTIL catheter is in the abscess.
01:49Now, we are going to take out the stilet.
01:52Now, we are going to connect it with the Euroback so that the abscess keeps on draining for the next 24 to 36 hours.
02:13Euroback connect to the earoback so that we will do the plastering of the catheter.
02:20So, skin and catheter we will do the plastering of the catheter.
02:36Now, we will give the instructions to the patient.
02:39If there is any problem, contact the facility.
02:44We have confirmed that the abscess is draining nicely.
02:50After the procedure, we will confirm the position of the catheter.
02:55It is in the abscess core.
02:58Once it is confirmed, we discharge the patient.
