Innovative ‘heart failure at home’ service empowers cardiac patients

00:01Hello Sally. Hi thank you for coming today. Our Febris technology is a
00:07partnership with BT Innovation and the local trust and to allow us to provide
00:13a technological approach to allowing patients to manage their conditions. We
00:16began Febris in October 2024 and we wanted to provide patients with
00:21technology and equipment that allows them to monitor their condition at home
00:25empowering them to make changes to their management and also contact
00:29us if they have any concerns. Through the system we get twice weekly reports from
00:34each patient allowing them to report any signs that have developed, any new
00:39symptoms that have developed and we review this twice weekly and then can make
00:43changes to the patient management preventing them coming to hospital but
00:47also providing them with reassurance that we're able to monitor their condition.
00:51For those patients eligible for the service we would issue them with a kit
00:54that includes a mobile telephone device and blood pressure cuff, saturations probe
01:01and a set of scales and they all connect through Bluetooth technology and a data
01:06card which then allows data to be transmitted centrally back to Lagan Valley
01:10where we can review it. Within the Febris team my role would be to review the
01:14data twice a week and if there's any issues with the data I would then ring them and
01:20follow it up. If I'm worried about the patient and I'll bring them to clinic.
01:24It's actually very good, it's quickening up our process of filtering through the
01:29data and the data is presented on the scales and on charts and we can see the
01:34trend. It takes away the time involved in clinic appointments. We're finding our
01:40relationship with the patients is becoming better because they feel more at
01:45ease using this mechanism to present the data. It provides a lot of empowerment to
01:52the patient to allow them to look after their condition and through the technology
01:56with patients being able to report how they feel and provide messages to the
02:00team here and it makes them much more in control of the condition. The biggest key
02:04factor for me is watching the patients and you know jump on board and get
02:09involved with this. If patients can be managed in their own home or can be
02:12managed in an ambulatory centre that is our preference.
