Another woman in New South Wales has died in her own home. Allegedly murdered at the hands of her partner, police are investigating whether they could have done more to save her. They have launched critical investigation into why it took almost an hour to respond to the triple-zero call. The woman, believed to be in her 40’s, was alive when police arrived but died shortly afterwards. A 31-year-old man has been charged with murder.

00:00A woman is dead and a man in custody in what police are treating as another case of domestic
00:08violence. Forensics teams are piecing together just what happened at this home and casino
00:13in northern New South Wales. A neighbour called triple zero in the early hours of the morning.
00:18I didn't hear anything until about 1.20 in the morning.
00:24The response of emergency services is now in the spotlight. After it was revealed it
00:28took nearly an hour for them to react. Authorities say they received the triple zero call at
00:33around 1.30 this morning, but they didn't respond for nearly an hour. Then at 2.25am
00:40police say they acknowledged the call, arriving at the scene less than two minutes later.
00:45She was unconscious but breathing. Police assisted her, called for ambulance immediately.
00:51Ambulance arrived very shortly thereafter, but unfortunately her condition deteriorated
00:55and she died at the scene. It's believed she was in her 40s, while it's understood
01:00the man in custody was in a relationship with her. There were also signs she'd suffered
01:05head injuries. He's been charged with murder. I just wish we heard her earlier to be honest.
01:12Maybe we could have saved her from whatever was happening. What happened between about
01:181.30 when that triple zero call was made and 2.27 when police arrived at this address will
01:25be the subject of a critical incident investigation. A particular focus will be why a higher priority
01:31wasn't placed on the call. The delay in the timing of police acknowledging that call and
01:37attending the scene has given me enough concern that I've asked for an independent review.
01:43The 31-year-old man has been refused bail ahead of an appearance at the Lismore Bail
01:47Court tomorrow. The emergency response will also come under scrutiny, looking at whether
01:52more could have been done to save a woman's life.
