00:00 On August 1st, 2023, a mechanic was tasked with removing an exhaust from a pontoon pump

00:16 roof assembly. As a safer alternative to working at heights, the mechanic sought assistance

00:21 to lay the roof section over on its side to enable access to the exhaust from ground level.

00:29 Two qualified high-risk work licence holders, a crane operator and rigger, were subsequently

00:35 tasked with moving the roof section through use of a franner crane. Upon arriving at the

00:42 job location and discussing the task, the workers noted that the load had an obvious

00:49 weight imbalance to one side. The workers decided that they would use the franner crane

00:55 to first drag the roof section across the ground out of the way from other equipment.

01:03 Once the load was clear, it was intended that they would use the crane and the weight bias

01:08 of the roof to assist in laying it down on its heavier side. It was anticipated that

01:15 this could be achieved without the need for lifting the heavier side from the ground.

01:22 Ten millimetre chains were then attached from the franner crane to the two thinner legs

01:27 located on the lighter side of the load. Once ready, the lighter side of the load was raised

01:34 slightly with the heavier side remaining in contact with the ground. The rigger then signalled

01:42 for the crane operator to commence reversing the crane in order to drag the load away from

01:48 other equipment. During this stage, the rigger was pulling at one of the roof section legs

01:56 in attempts to assist in the movement of the load across the ground. The load had been

02:09 moved across the ground approximately one metre, at which point the load became unbalanced

02:15 and broke holding contact with the ground. The unbalanced load then become uncontrolled,

02:25 swinging and striking the rigger, causing serious leg injuries. The other workers then

02:31 attended to the injured rigger, initiating site emergency procedures and calling for

02:36 urgent medical assistance. In conclusion, sites should undertake a review

02:52 of fatal hazard controls and verification processes associated with lifting, including

02:58 not working within exclusion zones and the use of tag lines when appropriate. It is also

03:03 recommended that Safety Bulletin 199, updated and published 7 August 2023, be circulated

03:12 to all coal mine workers.
