00:04 she wasn't ventilating properly.

00:06 You inserted the tube too deep

00:08 into her right main stem bronchus of her injured lung.

00:13 We had no way of knowing how long she was hypoxic

00:17 before we gave her the EEG.

00:20 ♪ ♪

00:27 I drilled a perfect burr hole into her skull

00:35 on the side of the road in the middle of the night,

00:37 and I--I--I screw up her intubation.

00:40 Yes. It was tough conditions for anyone, resident or not.

00:44 Any one of us could have made the same mistake.

00:54 I had a woman come into the ER once.

01:05 A mother, 51 years old.

01:08 Francine Parker.

01:11 She had two boys.

01:13 She had a cough.

01:16 A bad one, but a cough.

01:20 I checked her out, gave her some antibiotics,

01:22 sent her home.

01:23 Six hours later, she was rolled into the ER.

01:27 She had had a heart attack, and I missed it,

01:29 and...she died.

01:38 goes by when I don't think about her.
