On Friday 18th April a huge serac on the west shoulder of Mount Everest collapsed, and swept across the Khumbu icefall, just below Camp 1, at an altitude of roughly 5800m.

The incident occurred extremely quickly and without warning, meaning that those in the path of the falling debris were unable to get out of the way. So far, 16 fatalities have been confirmed, all Nepalis working for commercial expeditions.
The serac in question has been the cause of concern in the past, with veteran Himalayan guide Russell Brice cancelling his 2012 Everest expedition due to concerns about it's stability. The serac has now collapsed 3 out of the past 4 seasons.
The various expeditions are currently weighing up whether to continue with their summit bids but everyone is on hold while they Nepali Ministry of Tourism decides what to do about 13 demands which have been sent to them by the Sherpas currently working on the mountain.

The demands include pay outs to the families of the deceased, increasing insurance for Sherpas working on the mountain, and a promise not to penalize Sherpas who refuse to go into the icefall this year.
No doubt the debate about the commercialization of mountaineering, and in particular Everest, will be fired up again by this incident but for the meantime I'd like to send the condolences of everyone at Epictv to those affected by this tragedy. A Sherpa I worked with on 2 expeditions to Nepal, Pasang Karma, was killed in the incident and my thoughts are with his family and friends today.
