Shane Douglas breaks Gary Wolfe's neck

On July 13, 1996, Shane Douglas was in a 4-Way elimination match for the ECW Television title against 2 Cold Scorpio, Chris Jericho, & Pitbull #2.
At one point, when Douglas was looking to be victorious, Pitbull #2's partner, Gary Wolfe, interfered in the match but to no avail. Douglas snuck up behind Wolfe and delivered a single-arm DDT on top of the ECW television title. At first glance, everything seemed to go off without a hitch, but as the two men explain in the video, that was not the case.
Prior to the match, Douglas claims in the FOREVER HARDCORE documentary that he told Gary Wolfe all day that Gary needed to land flat, pancake style, in order to protect himself from the DDT. Shane claims that Wolfe was animate about wanting the spot to look good and wanted to land in an almost spike position.
The end result was Gary Wolfe breaking his C1 vertebre in half.
Wolfe makes no mention of this in the FOREVER HARDCORE. In fact, he is un forgivingly bitter towards Douglas, seemingly blaming the incident entirely on the Franchise as if Douglas had done it on purpose.
After Douglas had broke Wolfe's neck, ECW turned the incident into a real angle. ECW booker Paul Heyman wanted Douglas to cut several promos mocking Wolfe's injury and basically cripples in general by clamoring over the ability to swim or have Douglas cut vignettes in wheelchairs for the next few weeks. Douglas and Francine both seemed turned-off at the idea, despite having done them anyway.
The entire incident would eventually lead to an unprecedented riot at the ECW arena.
